Vitreoretinal surgical outcomes in intraocular foreign bodies

Abstract
Purpose: To determine prognostic factors and visual outcomes of cases with intraocular foreign bodies (IOFBs). Materials and Methods: Sixty-six eyes of sixty-six patients who presented with penetrating ocular trauma and retained intraocular foreign body to the Department of Ophthalmology Dokuz Eylul University School of Medicine comprised the study group. The patients were evaluated in terms of visual outcomes and prognostic factors. Associated preoperative findings were traumatic cataract (74.2%), intravitreal hemorrhage (37.9%), retinal detachment (13.6%) and endophthalmitis (12.1%). Results: Following pars plana vitrectomy and the removal of the intraocular foreign body, visual acuity improved in 32 cases (48.5%), did not change in 18 cases (27.3%), and decreased in 16 cases (24.2%) cases. Mean follow-up was 16.4±13.2 months (6- 69 months). We observed retinal detachment in nine cases (13.6%), phthisis bulbi in six cases (9.1%), secondary glaucoma in three cases (4.5%), and macular scar formation in two cases (3.1%) cases postoperatively. One patient (1.5%) developed sympathetic ophthalmia. Conclusion: Pars plana vitrectomy is found to be an effective therapeutic approach in removing intraocular foreign bodies, but preoperative and postoperative complications and timing of surgery determine the visual outcome and functional success.

Amaç: Göz içi yabancı cisim yaralanması olan hastalarda görsel sonuçları ve prognostik faktörleri değerlendirmek. Gereç ve Yöntem: Şubat 1998-Temmuz 2005 tarihleri arasında Dokuz Eylül Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalına delici göz yaralanması ve göz içi yabancı cisim nedeniyle başvuran 66 hastanın 66 gözü çalışmaya dahil edildi. Hastalar prognostik faktörler ve görsel prognoz açısından değerlendirildi. Cerrahi öncesi dönemde en sık eşlik eden bulgular travmatik katarakt (%74.2), intravitreal hemoraji (%37.9), retina dekolmanı (%13.6) ve endoftalmi (%12.1) olarak saptandı. Bulgular: Pars plana vitrektomi, göz içi yabancı cisim çıkarılması uygulanan 66 hastanın ameliyat sonrası en son muayenelerinde en iyi düzeltilmiş görme keskinliği 32 (%48.5) hastada artmış, 18 (%27.3) hastada değişmemiş, 16 (%24.2) hastada azalmış olarak saptandı. Ortalama izlem süresi 16.4±13.2 aydı (6- 69 ay). İzlem döneminde dokuz hastada (%13.6) retina dekolmanı, altı hastada (%9.1) fitizis bulbi, üç hastada (%4.5) sekonder glokom, iki hastada (%3.1) maküler skar, bir hastada (%1.5) sempatik oftalmi gelişti. Sonuç: Pars plana vitrektomi ile göz içi yabancı cisim çıkarılması etkili bir yaklaşım olmakla birlikte ameliyat öncesi ve sonrası eşlik eden komplikasyonlar ve cerrahinin zamanlaması görsel prognozu ve fonksiyonel başarıyı etkilemektedir
