Central serous chorioretinopathy after epidural corticosteroid injection

Abstract
We describe a case of central serous chorioretinopathy (CSCR) after epidural corticosteroid injection for the treatment of lower back pain related to lumbar hernia. A 45-year-old man presented with blurry and decreased vision in both eyes. Bilateral (CSCR) with multiple focus and diffuse pigment epitheliopathy in the left eye was detected in the fundus examination. All patients receiving systemic corticosteroid treatment by any route should be warned about possible visual symptoms and ocular pathologies, and in that case they should refer to an ophthalmologist at that time. In patients having bilateral CSCR and/or diffuse pigment epitheliopathy, a careful history should be taken including epidural steroid injection in addition to systemic corticosteroid treatment.

Bu çalışmada lomber disk hernisi kökenli bel ağrısı nedeniyle epidural kortikosteroid enjeksiyonu uygulaması sonrasında gelişen santral seröz korioretinopatili (SSKR) bir olgu tartışılmaktadır. Her iki gözde bulanık ve az görme ile başvuran 45 yaşındaki erkek hastada yapılan fundus muayenesinde bilateral ve çok odaklı santral seröz korioretinopati ile sol gözde makulada diffüz pigment epitelyumyopati saptanmıştır. Herhangi bir yolla sistemik kortikosteroid tedavisi uygulanan bütün hastalar olası görsel şikayetler ve oküler patolojiler hakkında önceden uyarılmalı; bu takdirde mutlaka bir göz hekimine başvurmaları konusunda bilgilendirilmelidirler. Bilateral SSKR ve/veya beraberinde diffüz pigment epitelyopati saptanan olgularda sistemik kortikosteroid yanında epidural steroid enjeksiyon öyküsü mutlaka sorgulanmalıdır.
