Associations between optical coherence tomography findings and visual outcomes after epiretinal membrane surgery

Abstract
Purpose: To evaluate associations between optical coherence tomography (OCT) findings and visual outcomes after 23 gauge vitrectomy for diabetic patients with secondary epiretinal membrane. Materials and Methods: Forty-three eyes of 43 patients who were operated on with 23 gauge vitrectomy for ERM and diabetic macular edema with a period of at least 6 months’ follow-up were investigated. The effects of surgery on postoperative 6 month visual acuity (VA) and macular thickness changes, and the associations between preoperative OCT findings (central macular thickness (CMT), presence of subfoveal exudate and detachment, foveal cyst, pseudohole and vitreomacular traction, the pattern of membrane adherence) and postoperative VA improvement and CMT change were studied. Results: Visual acuity improved 0.30 logMAR unit and was better in 39 eyes, and less than 0.30 logMAR unit in 4 patients. There were no significant associations between preoperative CMT, presence of subfoveal exudate and detachment, foveal cyst, pseudohole, vitreomacular traction, and VA change (p>0.05). When the same parameters’ effects were evaluated on CMT change, significant associations only with foveal cyst and preoperative CMT were detected. A significant difference was detected between the diffuse adherence pattern and mixed adherent patterns of ERMs on postoperative VA improvement (p=0.05). There were no significant associations between postoperative CMT change and VA change (p=0.35). Conclusion: 23 Gauge PPV provided 90.7% effective VA improvement in cases of diabetic maculopathy with ERM. No significant associations were detected between postoperative CMT change and VA change. Preoperative CMT and presence of foveal cyst affected postoperative CMT change significantly. In patients with ERM with both diffuse and focal adherence pattern the VA improvement was better than that in patients with diffusely adherent membranes.

Amaç: 23 gauge (G) pars plana vitrektomi (PPV) uygulanan diyabetik sekonder epiretinal membran (ERM)’lı olgularda optik koherens tomografi (OKT) bulguları ile ameliyat sonrası görsel prognoz arasındaki ilişkiyi değerlendirmek. Gereç ve Yöntem: Diabetik makula ödemine eşlik eden ERM’li olgulardan 23 G PPV uygulanan ve en az 6 aylık takibi olan 43 hastanın 43 gözü incelendi. Cerrahinin ameliyat sonrası 6. aydaki GK ve makula kalınlık değişimi üzerine etkisi; ameliyat öncesi OKT bulgularının (merkezi makula kalınlığı (MMK), subfoveal eksuda ve dekolman, foveal kist, yalancı delik, vitreomaküler traksiyon (VMT) varlığı, ERM yapışma tipi) ameliyat sonrası GK kazanımı ve makula kalınlık değişimi üzerine etkisi incelendi. Bulgular: Olguların 39’unda GK’de 0.30 logMAR unit ve üstü, 4’ünde 0.30 logMAR unit altı düzelme tespit edildi. Ameliyat öncesi OKT bulgularından MMK, subfoveal dekolman ve eksuda, foveal kist, VMT ve yalancı delik varlığı ile GK değişimi arasında anlamlı bir ilişki tespit edilmedi (p>0.05). Aynı parametrelerin MMK değişimi üzerine etkisi incelendiğinde foveal kist ve ameliyat öncesi MMK ile anlamlı ilişki saptandı. Diffüz yapışma paternine sahip ERM’lerde GK kazanımı ile karma yapışma paternine sahip olgularda ameliyat sonrası GK değişimi arasında anlamlı fark tespit edildi (p=0.05). Ameliyat sonrası MMK değişimi ile cerrahi ile sağlanan GK değişimi arasında anlamlı bir ilişki saptanmadı (p=0.35). Sonuç: ERM ile birlikte olan diyabetik makulopatili olgularda 23 G PPV %90.7 oranında etkili GK kazanımı sağlamıştır. Ameliyat öncesi OKT ile saptanan MMK’daki azalma ile ameliyat sonrası GK kazanımı arasında ilişki saptanmamıştır. Ameliyat öncesi MMK ve foveal kist varlığı ameliyat sonrası MMK değişimi üzerine anlamlı etkiye sahiptir. Diffüz ve fokal yapışıklığın birlikte görüldüğü membranlarda yalnız diffüz yapışıklık gösterenlere göre GK kazanımı daha fazla olmaktadır.
