Plasma and vitreous homocysteine levels in type 2 diabetic patients with proliferative retinopathy

Abstract
Purpose: To investigate comparatively plasma and vitreous homosistein (Hcy) levels between the patients with proliferative diabetic retinopathy and the patients with nonproliferative ocular disease. Material and Methods: Plasma and vitreous samples were obtained simultaneously at the time of vitreoretinal surgery from 16 type 2 diabetic patients with proliferative retinopathy and 13 nondiabetic patients with nonproliferative ocular diseases. Hcy levels were determined by high-performance liquid chromatography. Results: The vitreous and plasma Hcy concentrations for the diabetic group were (median;range) 1.4 mol/L (0.25-4.3) and 19.55 mol/L (1.8-39.8), respectively. The vitreous and plasma Hcy concentrations for the control group were (median; range) 1.8 mol/L (0.1-3) and 4 mol/L (2-17.2), respectively. Plasma Hcy concentrations for the diabetic group were significantly higher than those for the control group (p=0.004). A remarkable positive correlation between vitreous and plasma Hcy concentrations was present only for the diabetic group (Spearman’s, rho=0.487; p=0.056). Conclusion: Plasma Hcy levels in the patients with PDR were higher than control group. There was no difference of Hcy levels between diabetic and control groups probably due to no severe breakdown of the blood-retina barrier.

Amaç: Proliferative diyabetik retinopatisi olan hastalarda plazma ve vitreus homosistein (Hcy) seviyelerinin diyabetik olmayan olgular ile karşılaştırılmalı incelemek. Gereç ve Yöntem: Plazma ve vitreus örnekleri vitreoretinal cerrahi esnasında Tip 2 diyabetli proliferatif retinopatisi olan 16 hastadan ve 13 diyabeti olmayan oküler hastalıklı olgulardan alındı. Hcy düzeyleri yüksek performanslı sıvı kromatografi ile belirlendi. Bulgular: Vitreus ve plazma Hcy düzeyleri diyabetik grupta (ortanca; minimum-maksimum), 1.4 (0.25-4.3), 19.55 (1.8- 39.8) mol/L kontrol grupta vitreus ve plazma Hcy seviyeleri 1.8 (0.1-3); 4 (2-17.2) mol/L idi. Plazma Hcy düzeyleri diyabetik grupta kontrol grubuna göre istatistikî olarak anlamlı derecede yüksekti (p=0.004). Diyabetik grupta vitreus ve plazma Hcy konsantrasyonlar arası pozitif belirgin bir korelasyon mevcuttu (Spearman’s, rho=0.487; p=0.056). Sonuç: Plazma Hcy seviyesi PDR’li hastalarda kontrol grubuna göre yüksekti. Diyabetik ve kontrol gruplarının vitreus Hcy seviyeleri arasında farklılık olmaması kan-retina bariyerinin ileri düzeyde bozulmamasına bağlı olabilir.
