A comparison study: EMG biofeedback, prolonged passive stretching and classical physical therapy in the management of adhesive capsulitis

Abstract

In this study, 45 patients with adhesive capsulitis were divided in to three groups randomly. Following the treatment of three groups with the same superficial and deep heat physical modalites, active ROM exercises combined with EMG biofeedback were performed by group A. Group B were treated by prolonged passive stretching using pulley traction and standart exercises were performed by group C. At the end of the therapy the muscle electrical levels in group A and B showed significant increase (p<0.001). improvement was better in group A (p<0.01). At the end of the treatment, the restricted shoulder movements and pain improved significantly in group A and B (p<0.001). There was no stastical difference about passive and active joint range of motion and pain improvement (p>0.05), except active abduction movement which has greater improvement in group A (p<0.05). The decrease in pain scores at the end of the treatment in group C was significant (p<0.001). But the improvement in range of motion and muscle electrical activity was less significant than the other two groups (p>0.01). It was concluded that besides its other effects, EMG biofedback, for its superiority in arranging muscle electrical activity, is the most effective treatment method.

Özet

Bu çalışmada 45 adesiv kapsülit olgusu rastgele seçimle 3 ayrı gruba ayrılarak aynı yüzeyel ısıtıcı ve derin ısıtıcı fizik tedavi ajanlarını takiben, A grubuna EMG biofeedback eşliğinde aktif ROM egzersizleri, B grubuna pulley fraksiyonu kullanılarak uzun süreli pasif germe ve C grubuna ise standart egzersizler uygulandı. Tedavi sonunda A ve B' gruplarında kas elektriksel aktivite değerlerinde istatistiksel olarak çok anlamlı iyileşmeler bulundu (p 0.05). C grubunda tedavi sonu ağrı skorlarındaki azalma çok anlamlı bulundu (p<0.001). Ancak eklem hareket açıklığı ve kas elektriksel aktivitesinde elde edilen iyileşme diğer iki gruba göre daha az anlamlıydı (p<0.01). EMG biofeedback uygulamasının diğer etkilere ilaveten kas elektriksel aktiviteierinin düzenlenmesindeki üstünlüğü nedeniyle en etkin tedavi yöntemi olduğu sonucuna varıldı.
