Ulnar translation of the scaphoid bone in rheumatoid arthritis

Abstract
Ulnar carpal translation is a well recognized complication of the rheumatoid arthritis (RA). In this study we measured the distance between the os trapezium and radius known as scaphoid distance. We observed ulnar translation of the scaphoid bone resulting in _the shortening of the scaphoid distance in RA patients. Measurement of this distance was performed in 77 patients (154 hands) in supination with permanent angle. The patients were divided into two groups. Group 1 (control group) consisted of 25 patients who were referred to the radiology department following trauma. In Group 2, there were 52 patients with RA. Group 2 was divided into two subgroups. Group 2A included 21 patients without bone erosion, while group 2B included 31 patients with bone erosions. We observed that patients suffering from RA with bone erosions showed the smallest scaphoid distance. The ratio between the distance from distal radius to the most distal point of the third metacarpal bone and scaphoid distance was called carpometacarposcaphoid index. The average indexes were: 7.23±1.45 in the control group; 10.81+2.25 in Group 2A: 31.32±17.99 in Group 2B. The results were statistically significant. We think that these mesaruments are easy to perform and may be helpful in the early X ray diagnosis of the RA. Koy words: Rheumatoid arthritis, scaphoid distance, carpometacarposcaphoid index.

Karpal kemiklerin ulnar tarafa yer değiştirmesi romatoid artrit'in (RA) iyi tanımlanmış bir komplikasyonlarındandır. Bu çalışmada skafoid mesafe (SM) olarak isimlendirilen, trapezium ve radius arasındaki uzunluk ölçülmüş ve RA'lı hasta grubunda SM'nin kısalması ile sonuçlanan skafoid kemiğin ulnar yöne yer değiştirmesi gözlenmiştir. SM ölçümü 77 hastada (154 el) supinasyon pozisyonunda ve sabit bir açıda gerçekleştirilmiştir. Hastalar kontrol grubu ve RA'lı hastalar olmak üzere 2 gruba ayrılmıştır. Grup 1 (kontrol grubu) radyoloji servisine travmayı takiben gönderilmiş 25 hasta, Grup 2 ise 52 RA'lı hastadan oluşmuştur. Grup 2 yi oluşturan RA'lı hastalar kemik erozyonu içermeyen (Grup 2A: 21 hasta) ve kemik erozyonu içeren (Grup 2B: 31 hasta) hastalar olmak üzere 2 subgruba ayrılmış, ve en kısa SM erozyonlu grupta tesbit edilmiştir. Radiusun distal ucundan 3. metakarpal kemiğin distaline kadar olan mesafenin SM'ye oranı karpometakarposkafoid indeks (KMSI) olarak değerlendirilmiştir. Ortalama indeks değerleri kontrol grubunda: 7.23±1.45, Grup 2A da: 10.81+2.25, Grup 2B de:31.32±17.99 olarak bulunmuş ve sonuçlar istatistiksel olarak anlamlı yorumlanmıştır. Çalışma sonuçlarımız bu ölçüm tekniklerinin uygulanılması kolay ve RA'nın erken radyolojik tanısında yardımcı olabileceğini destekler niteliktedir.
