Assessment of autonomic function in patients with rheumatoid arthritis

Abstract
The existence of autonomic neuropathy in rhematoid arthritis (RA) can increase the morbitidy and mortality. The purpose of our study is to evaluate the autonomic nervous system involvement in the patients with RA. We performed sympathetic skin response (SSR) and R-R interval variation (RRIV) tests in 25 patients and 25 healthy controls to assess the autonomic function. Of 25 patients with RA, 3 had SSR and 5 had RRIV abnormalities. Only 2 patients had clinical dysautonomia. 3 of 25 patients also showed the electrophysiological evidence of peripheral neuropathy. This study has showed that autonomic nervous system invorvement is frequent in RA. SSR and RRIV, both of which can easily be performed in the electromyography laboratory, are helpful in detection of autonomic neuropathy.

Romatoid Artritte (RA) otonomik nöropati mevcudiyeti morbitide ve mortaliteyi arttırabilir. Çalışmamızın amacı RA'Iİ hastaları otonom sinir sistemi tutulumu yönünden değerlendirmektir. Bu amaçla 25 RA'Iİ hasta ve 25 sağlıklı bireye otonomik fonksiyonları araştırmak için sempatik deri yanıtı (SSR), ve R-R interval değişimleri (RRIV) testleri uygulanmıştır. 25 RA'Iİ hastanın 3'ünde SSR, 5'inde RRIV patolojisi tesbit edilirken sadece 2 hastada klinik disotonomi bulguları gözlenmiştir. Ayrıca RA'Iİ hastaların 3'ünde elektrofizyolojik olarak periferal nöropati bulguları saptanmıştır. Çalışmamız RA'Iİ hastalarda otonomik sinir sistemi tutulumunun oldukça sık olduğunu ve elektrofizyoloji laboratuarlarında kolayca uygulanabilecek SSR ve RRIV feslerinin otonomik nöropati tanısında yararlı yöntemler olduğunu göstermiştir.
