Abstract

CHRONİC FAMILIAL MEDITERRANEAN FEVER ARTHRITIS The arthritis of Familial Mediteranean Fever (FMF) presents as acute, self limited attacks of pain and swelling of one articulation and subsides less than a week. Rarely protracted articular attacks can occur especially in the knee and the hip joints. The main clinical observation is the absence of permanent sequel even in longstanding recurrent cases. Here we describe two patients with FMF who developed protracted articular attacks. These did not respond nonsteroidal anti inflammatory drugs, colchicine and intraarticular glucocorticoid treatment. Only synovectomy controlled their articular symptoms. As we observed in these two cases, arthritis of patients with FMF may show an indolent course that may not respond to any medical treatment and may need synovectomy for the relief of the symptoms,

Özet

Ailevi akdeniz ateşi (FMF) artriti akut, bir haftadan kısa süren, kendiliğinden geçen ağrı ve şişlik atakları şeklindedir. Nadiren özellikle diz ve kalça eklemlerinde kronikleşebilir. Asıl klinik gözlem uzun süreli tekrarlayan vakalarda dahi kalıcı sekel görülmemesidir. Burada kronik artritli iki FMF'li hasta sunulmuştur. Bu hastalar non steroid anti inflamatuvar ilaçlara, kolşisin ve intraartiküler steroid tedavisine cevap vermemişlerdir. Eklem semptomları sadece sinovektomi ile kontrol altına alınmıştır. Bu iki olguda, FMF artritinin hiçbir medikal tedaviye cevap vermeyen kronik seyirli gidiş gösterebildiği ve semptomlarının ortadan kalması için sinovektomi gerekebildiği görüldü.
