Analysing prescriptions in terms of prescribing indicators: A Case Study in Ankara

Abstract
Irrational, ineffective and economically inefficient use of drugs is a common problem throughout the world. While inappropriate use of drugs reduces the quality of medical care, it also leads to a waste of resources. Prescribing indicators developed by WHO is often used in researching of drug use. In this study, drug use is evaluated in Ankara province by using prescribing indicators. 1770 of all prescriptions has been analized. The results indicate that average number of drugs per prescription is 3,15, percentage of drugs prescribed by generic name is 19,21%, percentage of prescriptions with an antibiotic prescribed is 26,61 %, percentage ofprescriptions with an injection prescribed is 7,90% and percentage of drugs prescribed from essential drug list is 35,72%. The average number of drugs per prescription written by general practitioners at primary health units is higher. General practitioners prescribe more antibiotics than specialists and in primary health units'percentage of prescriptions with an antibiotic prescribed is higher. The percentage of drugs prescribed by generic name and drugs prescribed from essential drug list is very low. The use of injections is low in general. It was concluded that prescriptions are not written appropriate to prescription indicators.

İlaçların akılcı olmayan, etkisiz ve verimsiz kullanımı dünya genelinde görülen bir problemdir. İlaçların uygunsuz kullanımı sağlık hizmetlerinin kalitesini düşürürken kaynakların da boşa harcanmasına neden olmaktadır. İlaç kullanımı ile ilgili araştırmalarda DSÖ'nün geliştirmiş olduğu reçeteleme göstergeleri sıklıkla kullanılmaktadır. Bu araştırmada reçeteleme göstergeleri kullanılarak Ankara il merkezinde ilaç kullanımı değerlendirilmiştir. Bu amaçla 1770 reçete incelenmiştir. Verilerin analizi sonucunda; reçete başına düşen ortalama ilaç sayısı 3,15, jenerik isimle yazılan ilaç yüzdesi %19,21, antibiyotik içeren reçete yüzdesi %26,61, enjeksiyonluk ilaç içeren reçete yüzdesi %7,90, temel ilaç listesine göre reçete edilen ilaç yüzdesi %35,72 olarak bulunmuştur. Ortalama ilaç sayısı 1. basamak sağlık kurumlarında ve pratisyen hekimlerde daha yüksektir. Pratisyen hekimlerin uzman hekimlere göre daha yüksek oranda antibiyotik yazdığı ve 1. basamak sağlık kurumlarında da bu oranın yüksek olduğu belirlenmiştir. Jenerik ve temel ilaç listesine göre reçete edilen ilaç yüzdeleri düşük bulunmuştur. Enjeksiyonluk ilaç kullanımı genel olarak düşüktür. Reçetelerin genel olarak reçeteleme göstergelerine uygun yazılmadığı bulunmuştur.
