Relationship of diabetic retinopathy with haematologic parameters

Abstract

Purpose: To investigate the relationship between presence of diabetic retinopathy and haematologic parameters. Method : Sixty two patients with type II diabetes were divided into 3 groups as without retinopathy, non-proliferative diabetic retinopathy and proliferative retinopathy by ocular fundus examination. Afterwards, the following haematologic parameters were examined: haemoglobin, haematocrit, eritrocyte, leucocyte, mean corpuscular volume (MCV), fibrinogen, protrombin time (PT), activated partial tromboplastin time (APTT), protein S and protein C. Results were compared between groups by variance analyses. Results : Thirty-seven cases were determined as without retinopathy, 10 cases as with non-proliferative retinopathy, 15 cases as with proliferative retinopathy. Eritrocyte counts were significantly higher in cases without retinopathy (p=0.023). Haemotocrit and haemoglobin values showed a tendency to decrease in cases with retinopathy compared to cases without retinopathy, but this was statistically insignificant (p>0.05). Other haematologic parameters were not statistically different between groups (p>0.05). Conclusions: Results of the study suggested that diabetic retinopathy development was not associated with defects in plasma coagulation system or increases in serum viscosity. The significantly higher eritrocyte count in cases without retinopathy and the tendency of heamoglobin and haemotocrit values to decrease in cases with retinopathy leads one to think that anemia may be a risk factor for retinopathy development.

Amaç : Diabetik retinopatinin varlığı ve evresiyle hematolojik parametrelerin ilişkisinin araştırılması. Yöntem : Tip II diabetli 62 hasta göz dibi muayenesi ile retinopatisiz, nonproliferatif diabetik retinopatili ve proliferatif retinopatili olmak üzere 3 gruba ayrıldı. Ardından hastalarda şu hematolojik parametreler incelendi: hemoglobin, hematokrit, eritrosit, lökosit, ortalama korpüsküler volüm (MCV), fibrinojen, protrombin zamanı (PT), aktive parsiyel tromboplastin zamanı (APTT), protein S, protein C. Sonuçlar varyans analiziyle gruplar arasında karşılaştırıldı. Bulgular : Otuz yedi olgu retinopatisiz, 10 olgu nonproliferatif retinopatili, 15 olgu proliferatif retinopatili olarak belirlendi. Retinopatisiz olgularda eritrosit sayısı anlamlı düzeyde daha yüksekti (p=0.023). Retinopatili olgularda hemoglobin ve hematokrit değerlerinde retinopatisiz olgulara oranla düşme eğilimi izlendi, ancak istatistiksel olarak anlamlı değildi (p>0.05). Diğer hematolojik parametreler gruplar arasında anlamlı farklılık göstermiyordu (p>0.05). Sonuç : Bu çalışmada diabetik retinopati gelişimi ile plazma koagulasyon sisteminde bozukluk ve serum viskositesinde artış arasında ilişki kurulamamıştır. Retinopatisiz olgularda eritrosit sayısının anlamlı olarak daha yüksek olması, retinopatili olgularda hemoglobin ve hematokrit değerlerinde düşme eğilimi olması aneminin retinopati için bir risk faktörü olabileceğini düşündürmektedir.
