Experimental prophylaxis of Staphylococus aureus endophthalmitis after vitrectomy clinical and histopathologic evaluation

Abstract

Purpose: The aim of this study was to investigate the prophylactic efficacy of vancomycin in irrigating solution during vitrectomy to prevent Staphylococus aureus (S.aureus) endophthalmitis. Materials and Methods: This study was carried out on one eye of 36 healthy pigmented rabbits each weighing 2000-3000 g. The rabbits were divided into six groups each containing six rabbits. Vitrectomy was performed to all of the rabbits. Vitrectomy using 100 ml of BSS (balanced salt solution) irrigating solution with or without vancomycin (20$mu$/ml) was followed by intravitreal injection of S. aureus. In Group 1, BSS was used , followed by injection of 1.000 colony forming units (CFU) S. aureus; In Group 2, BSS plus vancomycin (VBSS) was used, followed by intravitreal injection of 1.000 CFU S. aureus; In Group 3, BSS was used followed by injection of 2.000 CFU S. aureus; In Group 4, VBSS was used followed by injection of 2.000 CFU 5.aureus; In Group 5, BSS was used followed by injection of 4.000 CFU S. aureus; In Group 6,VBSS was used followed by injection of 4.000 CFU S. aureus. The eyes were examined clinically for 5 days after surgery. At the end of fifth day, histopathologic examination was performed. Results: Endophthalmitis developed in all eyes in groups 1, 3, 5 (not given vankomycin) and Group 6 (was given vancomycin). Endophthalmitis did not developed in groups 2 and 4. The concentration of 20 $mu$/ml vancomycin significantly diminished the intraocular inflammation after injection of 10OO and 2000 CFU S.aureus(p<0.01). Vancomycin in the irrigating solution at this concentration did not significantly effect incidence of endophthalmitis when 4.000 CFU S. aureus was injected (p>0.05). Conclusion: In this animal model, vancomycin in a concentraiton of (20$mu$/ml) appears to prevent postvitrectomy S.aureus endophthalmitis when given in the infusion solution.

Özet

Amaç: Bu çalışmanın amacı, vitrektomi sonrası Stafilokokus aureus (S.aureus) endoftalmisinin deneysel profilaksisinde infüzyon sıvısı içerisinde van komisi n kullanımının etkinliğini araştırmaktır. Gereç ve Yöntem: 2000-3000 gr ağırlığında 36 adet sağlıklı pigmentli tavşan, her bir grupta 6 denek olacak şekilde 6 gruba ayrılmıştır. Tüm deneklerin bir gözüne vitrektomi uygulanmış olup, vitrektomi sırasında vankomisin (20$mu$/ml) içeren veya içermeyen 100 ml DTS (dengeli tuz solüsyonu) infüzyon solüsyonu kullanılarak operasyon sonunda S.aureus enjeksiyonu yapılmıştır. Grup 1, DTS kullanılmış olup takiben 1.000 forming unit (CFU) S.aureus; Grup 2, DTS içerisinde 20u/ml vankomisin (VDTS) kullanılmış olup, takiben 1000 CFU S. aureus, Grup 3, DTS kullanılmış olup, takiben 2.000 CFU S.aureus; Grup 4, VDTS kullanılmış olup, takiben 2.000 CFU S.aureus; Grup 5, DTS kullanılmış olup, takiben 4.000 CFU S.aureus;Grup 6, VDTS kullanılmış olup, takiben 4.000 CFU S. aureus enjeksiyonu yapılmıştır. Denekler cerrahi sonrası 5 gün süre He klinik takibe alınıp, 5. günün sonunda histopatolojik incelemeye tabi tutulmuştur. Bulgular: Vankomisin kullanılmayan grup 1, 3, 5 ve vankomisin kullanılan grup 6' da tüm gözlerde endoftalmi gelişti. Grup 2 ve 4' de ise endoftalmi gözlenmedi, infüzyon solüsyonu içerisinde 20u/ml konsantrasyonda kullanılan vankomisin, 1.000 ve 2.000 CFU S.aureus ile oluşan göz içi enflamasyonunu anlamlı derecede (p 0.05). Sonuç: Vitrektomi sonrasında S. aureus ile olabilecek endoftalmi profilaksisinde infüz-yon solüsyonu içerisinde (20$mu$/ml) vankomisin kullanımının etkili olduğu sonucuna varıldı.
