Anterior hyaloidal fibrovascular proliferation after vitrectomy in cases with proliferative diabetic retinopathy

Abstract
Purpose: To determine the frequency of anterior hyaloidal fibrovascular proliferation(AHFVP) after pars plana vitrectomy (PPV) in patients with proliferative diabetic retinopathy; to evaluate the properties of these patients and the anatomical, functional results of surgical treatments. Materials and Methods: The records of patients in which PPV was performed due to tractional retinal detachment, intravitreal hemorrhage between January 2001-April 2004 were retrospectively reviewed. The demographic properties, the type, the duration of diabetes, postoperative AHFVP development time and the anatomical, functional results of surgical treatments were evaluated. Results: Out of 201 eyes, AHFVP developed after a mean of 2.3 (1-5) months postoperatively in 9 eyes (4.4%) of 7 patients (male/female:4/3) in which 5 out of 7 were type I DM with a mean age of 38,2 (24-69). 4 patients had diabetic nephropathy, anemia. Median BCVA was hand motion (light perception- 2m counting fingers). Only equatorial cryotherapy (EK) was performed in 1 patient; in 8 eyes PPV with intensive endolaser (n:8), lens extraction (n:5), scleral buckling (n:1), EK (n:6) were performed. At the end of a mean follow-up time of 7.1 (1-12) months, retina was attached in 5 and detached in 4 eyes. Phtysis developed in 1 eye. Postoperative median BCVA was light perception; 2 eyes lost light perception. Conclusion: With surgical treatment, anatomical success was achieved in 55% of eyes with AHFVP; however, visual aquities remained at low levels. Postoperative close followup of patients who carry high risks of developing AHFVP is very important for the early diagnosis and treatment. Tight regulation of diabetes and especially the prevention of anemia is necessary along with intensive surgical treatment

Amaç: Proliferatif diyabetik retinopati (PDR) nedeniyle pars plana vitrektomi (PPV) uygulanan olgularda postoperatif dönemde ön hyaloidal fibrovasküler proliferasyon (ÖHFVP) gelişim sıklığını ve bu olguların özelliklerini belirlemek; cerrahi tedavilerin anatomik ve fonksiyonel sonuçlarını incelemek. Gereç ve Yöntem: Ocak 2001-Nisan 2004 tarihleri arasında traksiyonel retina dekolmanı, vitreus hemorajisi nedeniyle PPV uygulanan olguların kayıtları retrospektif olarak değerlendirildi. ÖHFVP gelişen olgularda demografik özellikler, diyabet tipi ve süresi, postoperatif ÖHFVP gelişim zamanı ve ÖHFVP'nin cerrahi tedavisinin anatomik, fonksiyonel sonuçları incelendi. Bulgular: 201 gözden, ortalama yaşı 38,2 (24-69), 5'i tip I DM'li 7 hastanın (erkek/kadın:4/3) 9 gözünde (%4.4) postoperatif ortalama 2.3 (1-5) ay sonra ÖHFVP gelişti. Dördünde diyabetik nefropati, anemi mevcuttu. Olguların medyan en iyi görme keskinliği (EİGK) el hareketi (ışık hissi- 2 mps) seviyesindeydi. 1 olguya sadece ekvatoryal kriyoterapi (EK); 8 göze ise PPV'yle birlikte yoğun endolaser (n:8), lens ekstraksiyonu (n:5), skleral çökertme (n:1), EK (n:6) uygulandı. Postoperatif ortalama 7.1 (1-12) ay takip sonunda retina 5 gözde yatışık, 4 gözde dekoleydi. 1 gözde fitizis gelişti. Postoperatif medyan EİGK ışık hissiydi; 2 gözde ışık hissi kaybedildi. Sonuç: Cerrahi tedaviyle ÖHFVP'li gözlerin % 55'inde anatomik başarı sağlanmıştır, ancak görme seviyesi düşük seviyede kalmıştır. ÖHFVP gelişimi açısından yüksek risk taşıyan olguların postoperatif yakın takibi, bu ciddi komplikasyonun erken tanısı, tedavisi için çok önemlidir. Yoğun cerrahi tedavinin yanı sıra, diyabetin sıkı regülasyonu ve özellikle aneminin önlenmesi gereklidir.
