Posterior subtenon triamcinolone acetonide in the treatment of macular edema secondary to the retinal vein occlusion

Abstract

Purpose: To evaluate the effect of posterior subtenon triamcinolone acetonide for macular edema secondary to the retinal vein occlusion Materials and Methods: Twentyfour eyes of 24 patients with macular edema secondary to the retinal vein occlusion aged 45 to 82 years (average 62,5) was studied. Posterior subtenon injection of triamcinolone acetonide was applied to treat macular edema. The visual and anatomical responses were observed as well as complications related to the injection prosedure and triamsinolone acetonide medication. Patients was examined at first, third, and sixth months. Results: In fourteen eyes (58.3%) of 24 patients visual acuity increased with anatomical healing. In four eyes (16.6%), there was anatomical healing but visual acuity were stabile due to ischemic maculopathy. In two eyes (8.3%) visual acuity stabile due to macular edema. Visual acuity decreased in 4 eyes (16.6%) because of in one eye with reccurent branch retinal vein occlusion and three eyes with dense retinal hemorage. Anatomical and functional healing was observed in 19 eyes (79.1%) at the third month. At sixth month; visual acuity increased in 19 eyes (79.1%) and was stabile in 4 eyes (16.6%). Visual acuity decreased in 1 eye (4.1%) at the sixth month. Intraocular pressure was measured as 22 mmHg in one eye at the firsth month. Conclusion: Posterior subtenon triamcinolone acetonide injection is an effective therapeutic method in eyes with macular edema secondary to retinal vein occlusion. Visual prognosis may change with complications.

Özet

Amaç: Retina ven tıkanıklığına bağlı oluşan makula ödemi tedavisinde posterior subtenon triamsinolon asetonid uygulamasının etkinliğini değerlendirmek. Gereç ve Yöntem: Retina ven tıkanıklığı sonucu oluşan makula ödemi mevcut, yaşları 45 ile 82 arasında değişen (ortalama yaş; 62,5), 24 hastanın 24 gözü çalışmaya alındı. Vakalara makula ödemi tedavisi için posterior subtenon triamsinolon asetonid enjekte edildi. Tedavi etkinliği görme keskinliği ve anatomik iyileşme ile değerlendirildi. Ayrıca enjeksiyon veya triamsinolon asetonide bağlı gelişebilecek komplikasyonlar da takip edildi. Hastaların 1. 3. ve 6. aylarda kontrolleri yapıldı Bulgular: Birinci ayda 24 gözün 14'ünde (%58.3) görme keskinliğinde artış ile birlikte anatomik iyileşme saptanırken, 4'ünde (%16.6) anatomik iyileşmeye rağmen iskemik makulopati nedeniyle görme sabit kaldı. İki gözde (%8.3) ödemin sebat etmesi nedeniyle görme aynı kaldı. Dört gözde (%16.6) ise görme kaybı saptandı. Görme kaybı sebebi bir gözde tekrarlayan ven dal tıkanıklığı, üç gözde ise absorbe olmayan yoğun retina hemorajisi idi. Üçüncü ayda 19 gözde (%79.1) anatomik ve fonksiyonel iyileşme saptandı. Altıncı ayda; 19 gözde (%79.1) görme keskinliğinde artış saptanırken 4 gözde (%16.6) görme keskinliği sabit kaldı ve 1 gözde (%4.1) görme kaybı bulundu. Bir gözde birinci ayda göz içi basıncı 22 mmHg olarak ölçüldü. Sonuç: Retina ven tıkanıklığına bağlı makula ödemi oluşan gözlerde posterior subtenon triamsinolon asetonid makula ödemi azaltmada etkili gözükmektedir. Hastalardaki görsel prognoz; gelişebilecek komplikasyonlarla değişebilir.
