Effect of menopause on central retinal sensitivity

Abstract
Purpose: To asses the effect of menopause on retinal sensitivity using short wave automated perimetric (SWAP) parameters versus standard achromatic automated perimetric (SAP) parameters in premenopausal and postmenopausal healthy women. Materials and Methods: Left eyes of 32 postmenopausal women and 28 healthy controls with regular menses were randomly included in this study. Subjects were determined to be postmenopausal, based on blood Follicular Stimulating Hormone (FSH), Luteinizing Hormone (LH), Estradiol, Progesterone levels and clinical complaints by the Gynecology and Obstetrics Department. Short wavelength (Blue-on– yellow) automated perimetry (SWAP) and standard achromatic (White-on-white) perimetric analysis (SAP) were performed in both premenopausal and postmenopausal women. Visual field analysis were performed using Humphrey Visual Field Analyser 750i with full threshold, central 30-2 program. Results: The mean ages of premenopausal and postmenopausal women were significantly different (p=0.001). When the results of SWAP and SAP were analyzed, the mean deviation (MD) values of SAP and SWAP in postmenopausal group were significantly higher than premenopausal group (p=0.019) and there was no significance when the correction was adopted to age. The mean PSD and CPSD value with SWAP were significant (p=0.005; p=0.016), but with SAP were not significant when the postmenopausal group was compared with the premenopausal group (=0.102; p=0.275). Conclusion: Postmenopausal women demonstrate significant depression in visual field threshold sensitivity with SWAP tests and significant positive correlation between the changes of generalized mean retinal sensitivity in SWAP and estradiol levels. The findings of the present study demonstrated that SWAP tests may be more sensitive to determine sex hormone dependent changes of menopause.

Amaç: Menopozun retina duyarlılığı üzerindeki etkisinin menopoz öncesi ve menopoz sonrası olgularda santral görme alanında oluşabilecek değişiklikleri renkli ve renkli olmayan görme alanı testleri ile incelenmeyi amaçladık. Gereç ve Yöntem: Otuz iki menopoz sonrası ile yirmi sekiz menopoz öncesi menstrual siklusları düzenli olan bayanın rastgele bir gözleri (sol gözleri) çalışmaya dahil edildi. Olguların menopoz sonrası olduğuna kadın doğum kliniğinde yapılan muayene ve folikül uyarıcı hormon (FSH), Luteinize hormon (LH), Östrojen ve Progestron seviyelerine göre karar verildi. Kısa dalga boylu otomatik perimetre (SWAP) ve standart akromatik perimetre (SAP) ile analiz menopoz öncesi ve sonrası kadınlarda gerçekleştirildi. Görme alanı işlemi, Humphrey Visual Field Analyser 750i ile renkli (sarı zeminde mavi) ve renkli olmayan 30-2 santral eşik testi ile gerçekleştirildi. Bulgular: Menopoz öncesi ve menopoz sonrası gruplar arasındaki yaş farkı anlamlı idi (p=0.001). Renkli ve renkli olmayan görme alanı test sonuçları analiz edildiğinde ortalama sapma (MD) değerleri menopoz sonrası grupta, menopoz öncesi gruptan daha yüksek ve istatistikî olarak anlamlı iken (p=0.019, p=0.02), yasa göre düzeltildiğinde istatistiki anlamlılık bulunmadı. İki grup arasında ortalama pattern standard sapma (PSD) ve düzeltilmiş pattern standard sapma CPSD değerleri renkli görme alanı testinde anlamlı farklılık göstermesine karşın (p=0.005; p=0.016), renkli olmayan görme alanı testinde istatistiksel farklılık tespit edilmedi (p=0.102; p=0.275). Sonuç: Renkli görme alanı testinde menopoz sonrası kadınların görme alanlarında belirgin depresyon görülmüş olup, generalize retina duyarlılığındaki değişim ile östrojen seviyeleri arasında belirgin artış gösteren ilişki tespit edilmiştir. Renkli görme alanı testi, menopozla ortaya çıkan cinsiyet hormon değişikliklerinin retinal duyarlılık üzerindeki etkilerini belirlemede daha etkin bir metodudur.
