Giant cell arteritis diagnosed with combined anterior ischemic optic neuropathy and cilioretinal artery occlusion

Abstract
A 65-year-old female was referred with a decrease in vision in the left eye to our clinic. Patient was managed for the diagnosis of acute renal failure in the internal medicine clinic. Visual acuity was hand motion in the left eye with a 2+ relative afferent pupillary defect and markedly decreased color vision. Fundus examination revealed optic disc edema as well as a cilioretinal artery occlusion. The patient denied any other ocular or systemic symptoms related to giant cell arteritis. Biopsy of the superficial temporal artery confirmed the diagnosis of giant cell arteritis. Pulse steroid treatment (1000 mg/day) was performed for three days before the biopsy was resulted. Treatment was contiuned with oral corticosteroid (1mg/kg). The retinal and optic nerve edema was improved at 2 month visit significantly. This case highlights the situation where clinical symptoms were not suspicious for giant cell arteritis but the presence of an anterior ischemic optic neuropathy and a cilioretinal artery occlusion was virtually pathognomonic for giant cell arteritis.
Özet
Dahiliye servisinde akut böbrek yetmezliği nedeniyle izlenen 65 yaşındaki bayan hasta sol gözde görme azalması şikayeti ile kliniğimize refere edildi. Sol gözde görme keskinliği el hareketleri düzeyinde, +2 relatif afferent pupiller defekt ve belirgin azalmış renk görmesi mevcut idi. Fundus muayenesinde optik disk ödemi ve aynı zamanda siliyoretinal arter tıkanıklığı mevcut idi. Hastanın dev hücreli arterit tanısını destekleyecek başka sistemik veya oküler şikayeti yok idi. Yüzeyel temporal arterden yapılan biyopsi dev hücreli arterit tanısını doğrulamaktaydı. Hastaya biyopsi sonucu elimize ulaşmadan derhal pulse steroid (1000 mg/gün) tedavisi 3 gün boyunca uygulandı. Tedaviye oral kortikosteroid (1mg/kg) ile devam edildi. 2 ay sonraki kontrolünde optik sinir ve retinal ödemin belirgin olarak düzeldiği gözlendi. Bu vaka, klinik şikayetlerin tam olarak dev hücreli arterit tanısını yansıtmadığı ancak anterior iskemik optik nöropati ve siliyoretinal arterin tıkanması birlikteliği ile kliniğimize başvurmuş dev hücreli arterit tanısı için patognomonik bir olgu olması nedeniyle özellik arzetmektedir.
