Long-term results of laser photocoagulation therapy in patients with branch retinal vein occlusion

Abstract
Purpose: To evaluate the visual outcome and secondary complications in the long-run in patients treated with laser photo- coagulation for branch retinal vein occlusion (BRVO). Materials and Methods: Medical records of the patients who were diagnosed with BRVO prior to optical coherence tomog- raphy era between May 1991 and May 2001 were evaluated. 139 eyes of 134 patients who were followed-up for two or more years were included into the study. Demographical and ophthalmological data of the patients were recorded. Results: The mean follow-up period was 4.4±3.3 (Range: 2-16) years. The mean visual acuity of the affected eyes was 0.69 log- MAR at the time of diagnosis and 0.58 logMAR at the last visit (p=0.012). Although retinal and optic disk neovascularization occured in 38 eyes (27%) of all eyes, none of the patients had experienced neovascular glaucoma. Macular edema was present in 69 eyes (49.6%). Visual acuity improved a mean of one line (Snellen chart and logMAR) in 69 of 139 eyes (50%) with BRVO treated with laser photocoagulation at the last follow-up visit. Improvement of two or more lines (Snellen chart) were observed in 49 of 139 (35%) eyes. Improvement of two or more lines were detected in 60 of 139 (43%) eyes by LogMAR chart. Conclusion: Permenant visual stabilization had been provided by laser photocoagulation in eyes with BRVO before the optical coherence tomography era.

Amaç: Retina ven dal tıkanıklığı (RVDT) nedeniyle lazer fotokoagülasyon tedavisi uygulanan hastalarda uzun takip sonra- sı görme keskinliği ve ikincil gelişen komplikasyonları değerlendirmek. Gereç ve Yöntem: Mayıs 1991-Mayıs 2001 tarihleri arasında optik koherens tomografi öncesi döneme ait RVDT tanısı alan hastaların dosyaları taranarak incelendi. 2 yıl ve üzeri düzenli takibi yapılan, 134 olgunun 139 gözü çalışmaya alındı. Hastaların demografik ve oftalmolojik bilgileri kaydedildi. Bulgular: Ortalama takip süresi 4.4±3.3 (2-16 yıl) yıl idi. Tutulan gözlerin tanı esnasında görme keskinliği ortalama 0.69 logMAR ünit iken, son muayenede ortalama 0.58 logMAR ünit olarak saptandı (p=0.012). Gözlerin 38inde (%27) retinal ve optik disk neovaskülarizasyonu gelişmesine karşın, hiçbir olguda neovasküler glokom gözlenmedi. Gözlerin 69unda (%49.6) maküla ödemi saptandı. Lazer fotokoagülasyon uygulanan RVDTli gözlerin son muayenesinde 139 gözün 69unda (%50) görme keskinliğinde (Snellen eşeli ve logMAR eşeli) ortalama bir sıra artış saptandı. 139 gözün 49unda (%35) 2 sıra ya da daha fazla (Snellen eşeli) görme artışı olduğu gözlendi. LogMAR eşeline göre 2 sıra ya da daha fazla görme artışı 139 gözün 60ında (%43) saptandı. Sonuç: Optik koherens tomografi öncesi dönemde, RVDT olan gözlerde uygulanan lazer fotokoagülasyon ile görmede kalıcı stabilizasyon sağlanmıştır.
