A case of tuberculosis meningitis and spondylodiscitis with ocular ınvolvement

Abstract

Ocular TB includes any infection in or around the eye caused by Mycobacterium tuberculosis or its related species. It may be either an active infection or an immunologic reaction, related to delayed hypersensitivity or an aseptic reaction. A 23-yearold female who had underwent surgery for thoracic spinal tuberculosis (TB), and was receiving treatment for concurrent TB meningitis was referred to our clinic with ocular pain and decreased visual acuity. Bilateral choroiditis with multiple tubercles was found in both eyes. Visual acuity and the ocular findings showed marked improvement with anti-tuberculous treatment. This is the only case with simultaneous occurence of spondylodiscitis, meningitis, and ocular tuberculosis. By means of the presented case, we discussed the findings and the management of this complicated situation.

Özet

Oküler Tüberküloz (TB), Mycobacterium tuberculosis’in, göz içinde ve çevresindeki dokularda meydana getirdiği granülomatöz bir enfeksiyondur. Bu olay aktif bir enfeksiyon olabileceği gibi, hipersensitivite veya aseptik reaksiyona bağlı immünolojik bir yanıt da olabilir. TB spondilodiskit tanısıyla ameliyat edilen ve aynı dönemde saptanan TB menenjite yönelik tedavi alan 23 yaşındaki bayan hasta, gözlerinde ağrı ve görme azalması yakınmaları nedeniyle kliniğimize sevk edildi. Her iki gözde yaklaşık 10’ar adet tüberkülle seyreden koroidit odakları saptandı. Başlanmış olan anti-TB tedavi ile görme keskinliği ve bulgular kısa sürede düzelme gösterdi. TB’a bağlı spondilodiskit, menenjit ve oküler tutulumun aynı anda görüldüğü literatürde benzer bir olgu bulunmaması nedeniyle bu zor olgudaki yaklaşım ve bulgular paylaşılmıştır.
