Evaluate effect of functional success existing photoreceptor layer defect in the treatment of age-related macular degeneration with ranibizumab

Abstract

Purpose: To evaluate effect of anti-VEGF (ranibizumab) treatment on functional success in age-related macular degeneration (AMD) existing photoreceptor layer defect. Material and Methods: Forty eyes of 37 AMD patients were included. Patients treated with 3 consecutive monthly intravitreal ranibizumab injections and reinjections according to PrONTO study. Before and after the treatment, all patients underwent complete ophthalmologic examination, including best-corrected visual acuity (BCVA) and time domain optical coherence tomography (OCT) scans. All patients had retinal fluid secondary to choroid neovascularization (CNV). Patients were divided in 2 groups according to existence of subfoveal inner and outer photoreceptor layer (IS/OS) defect. 18 eyes with subfoveal IS/OS junction defect included in Group 1, 22 eyes with intact IS/OS junction included in Group 2. Results: The mean age of 37 patients was 73.1±7.6 (55-85). The mean follow-up time was 24.44±9.94 (12-48) months. At the pre- sentation, visual acuity, central macular thickness (CMT) and thickest value of macula were statistically equal among 2 groups. The mean number of intravitreal ranibizumab injections was 9.4 in group 1 and 8.3 in group 2. After the treatment, there were a statisti- cally significant decrease in CMT and thickest value of macula in both groups. Final BCVA were similar to baseline acuity in group 1 and improved significantly in group 2 (p<0.001) Conclusion: AMD patients with IS/OS junction defect have low visual acuity impairment in spite of the anti-VEGF (ranibizumab) treatment.

Özet

Amaç: Yaşa bağlı maküla dejenerasyonunun anti-vasküler endotelyal growth faktör (anti-VEGF); Ranibizumab (Lucentis®; Genentech, South San Francisco, Calif., USA) ile tedavisinde fotoreseptör tabakasında defekt varlığının fonksiyonel başarı üzerine olan etkisini değerlendirmek. Gereç ve Yöntem: Yaşa bağlı maküla dejenerasyonu (YBMD) nedeniyle Pronto Protokolüne uygun olarak 3 doz ranibizumab uygulanan 37 hastanın 40 gözü tedavi öncesi ve sonrası en iyi düzeltilmiş görme keskinliği, fundus muayenesi ve time domain optik koherens tomografi (OKT) sonuçları açısından retrospektif olarak değerlendirildi. Hastalar koroid neovasküler membrana (KNVM) eşlik eden; retinal sıvı ile birlikte subfoveal iç fotoreseptör/dış fotoreseptör (IS/OS) hattı defekti olan (Grup 1) ve sadece retinal sıvısı olanlar (Grup 2) şeklinde iki gruba ayrıldı. Bulgular: Yaş ortalaması 73.1±7.6 (55-85 yaş) idi. Başvuru esnasında iki grubun görme değerleri, santral maküler kalınlık ve en kalın maküler yer değerleri arasında istatistiksel olarak anlamlı bir farklılık yoktu (p>0.05). Rutin 3 doz anti-VEGF ve ge- rektiğinde ek doz enjeksiyon tedavileri uygulandı. Hastaların takipleri boyunca Grup 1’de ortalama 9.4; Grup 2’ de ise ortalama 8.3 doz ranibizumab enjeksiyonu yapıldı. Hastalar 24.44±9.94 (12-48) ay takip edildi. Tedavi sonrasında her iki grupta da tedavi öncesine göre santral maküler kalınlık ve en kalın maküler yer değerleri istatistiksel açıdan anlamlı derecede azaldı. Görme keskinliği Grup 1’de değişmezken Grup 2’de istatistiksel açıdan anlamlı artış oldu (p<0.001). Sonuç: OKT’de fotoreseptör defekti mevcut olan hastalarda ranibizumab tedavisi ile görme keskinliğinde artış sağlanamamıştır.
