Visual Prognostic Factors with Spectral-Domain Optical Coherence Tomography in Cases with Epiretinal Membrane Surgery

Abstract
Purpose: To evaluate the significant prognostic factor on the final visual outcome for epiretinal membrane (ERM) surgery with spectraldomain optical coherence tomography (SD-OCT). Materials and Methods: Sixty-one eyes of 61 patients with epiretinal membrane that were followed-up in Ulucanlar Eye Training and Research Hospital between January 2010-June 2012 were respectively evaluated in this study. The best corrected visual acuity (BCVA logMAR), intraocular pressure, slit-lamp biomicroscopy, macular thickness and macular structure obtained with OCT images were evaluated at baseline and postoperative 6th months. Macular structures, including morphology of photoreceptor inner segment/outer segment junction, foveal contour, central macular thickness (CMT) were investigated as predictors visual outcome of the surgery. The cases were divided into two subgroups:Group-A was including patients with idiopathic ERM, group-B with secondary ERM. Results: The mean BCVA was 0.81±0.28,0.47±0.31 logMAR respectively at preoperative and postoperative 6th months for group-A, and 0.93±0.34, 0.65±0.32 logMAR for group-B. The difference between BCVA before, after surgery was statistically significant in both groups (for group-A p=0.000, for group-B p=0.001). The mean CMT was 468.33±111.30 at baseline, 398.57±78.60 µm at the 6th month for groupA. These values were 485.26±159.52, 326.31±94.58 µm for group-B, respectively. The reduction in the mean CMT was significant in both groups, (for group-A p=0.000, for group-B p=0.001). The association between macular structure and visual outcomes, the IS/OS junction disruption was statistically significant (p<0.05). Conclusions: Regularity and uninterrupted IS/OS junction on preoperative SD-OCT images was found to be an important predictor of better visual outcomes after epiretinal surgery

Amaç: Epiretinal membran (ERM) cerrahisi uygulanan hastalarda spektral domain optik koherens tomografi (SD-OKT) ile sonuç görme keskinliği üzerine etkin prognostik faktörlerin belirlenmesi. Gereç ve Yöntem: Ocak 2010-Haziran 2012 tarihleri arasında Ulucanlar Göz Eğitim ve Araştırma Hastanesi'nde ERM nedeni ile pars plana vitrektomi (PPV) uygulanan 61 hastanın 61 gözü geriye dönük değerlendirildi. En iyi düzeltilmiş görme keskinliği (EDGK, LogMAR), göz içi basıncı, biyomikroskobik bulgular, merkezi maküler kalınlık (MMK) ve maküler yapı görüntüleri, ameliyat öncesi ve ameliyat sonrası 6. ayda değerlendirildi. Fotoreseptör iç segment/dış segment bandı, foveal kontur yapısı ve MMK'nın sonuç görme keskinliğine olan etkisi araştırıldı. Primer ERM hastaları grup-A, sekonder ERM hastaları grup-B olarak isimlendirildi. Bulgular: Ameliyat öncesi ve sonrası 6. ay ortalama EDGK Grup-A için sırasıyla 0.81±0.28, 0.47±0.31 logMAR, grup-B için sırasıyla 0.93±0.34, 0.65±0.32 logMAR idi. Ameliyat öncesi ve sonrası EDGK değerleri karşılaştırıldığında cerrahi sonrası her iki grup için istatistiksel olarak anlamlı artış belirlendi (p=0.000). Ameliyat öncesi ortalama MMK grup-A için 468.33±111.30 µm, ameliyat sonrası 6. ayda 398.57±78.60 µm idi, Grup-B için sırasıyla 485.26±159.52, 326.31±94.58 µm idi. Her iki grup MMK açısından ameliyat öncesi ve sonrası değerlendirildiğinde, ameliyat sonrası maküler kalınlıkta istatistiksel olarak anlamlı azalma görüldü (grup-A için p=0.000, grup-B için p=0.001). Maküler yapı ile sonuç görme keskinliği arasındaki ilişkide, her iki grup için IS/OS bandı düzgünlüğü istatistiksel olarak anlamlı bulundu (p<0.05). Sonuç: Hem primer hem sekonder ERM cerrahisinde, ameliyat öncesi IS/OS bandının düzgünlüğü ve devamlılığı daha iyi görsel sonuçların elde edilmesinde önemli rol oynamaktadır
