The Magnitude of Posterior Segment Evaluation Using Optical Coherence Tomography Prior to Cataract Surgery

Abstract

More accurate estimation of postoperative visual prognosis before performing cataract surgery that is one of the most common surgical interventions all over the world, provides much more satisfaction for both patient and the physician. Hence, detailed ophthalmological examination of the patient who is scheduled for cataract surgery is crucial preoperatively. However, performing elaborate examination of posterior segment in cases especially with dense cataract or corneal abnormalities may be diffi cult and time consuming. Nevertheless accurately estimation of postoperative visual prognosis is more likely in such cases according to careful retinal examinations including optical coherence tomography scans. Obvious sharing of such fi ndings with the patients and their relatives in preoperative visits, and discussing about amendment of the surgical plans as well as the change of intraocular lens type in required cases have important impacts over postoperative satisfaction of both patient and the physician.

Özet

Tüm dünyada uygulanan en sık cerrahi girişimler arasında yer alan katarakt ameliyatı öncesinde ameliyat sonrasındaki görsel prognozun mümkün olduğunca doğru tahmin edilebilmesi hasta ve hekim açısından daha yüksek memnuniyet sağlamaktadır. Bu nedenle operasyon öncesinde ameliyatı planlanan hastanın detaylı oftalmolojik muayenesinin yapılması tıbbı bir gerekliliktir. Halbuki, özellikle yoğun katarakt mevcudiyeti ya da kornea problemi olan olgular için detaylı arka segment muayenelerini gerçekleştirmek zor olmakta ve zaman alabilmektedir. Bu hastalarda optik koherens tomografi taramalarını içeren titiz retina muayeneleri sonucu ameliyat sonrasındaki görsel prognozun doğrulukla tahmin edilebilmesi daha mümkün olmaktadır. Sonuçların ameliyat öncesi kontrollerinde hasta ve hasta yakınları ile açıklıkla paylaşılması ve gerekli durumlarda cerrahi planın ve hatta kullanılacak göz içi lens tipinin değiştirilmesi ile ilgili kararın detaylıca konuşulması, ameliyat sonrası dönemdeki hasta ve hekim memnuniyeti üzerine çok önemli etkileri bulunmaktadır.
