The value of new diagnostic criteria in spondylarthropathies

Abstract
The purpose of our study was to determine the value of the Amor and European Spondylarthropathy Study Group (ESSG) diagnostic criteria for spondylarthropathies. These criteria were proposed re¬cently and have been the subject of many studies. The study was performed in the Physical Medicine and Re¬habilitation Department of GATA Haydarpaşa Train¬ing Hospital. The patients were separated into two groups. The first group consisted of 157 patients diagnosed as spondylarthropathy. The other (i.e. control) group con¬sisted of 124 patients having various rheumatic diseases other than spondylarthropathy. To evaluate the sensitivity and specificity of these crite¬ria, 24 parameters were used. Furthermore, the perfor¬mance and the positive and negative predictive values of the two sets of criteria were evaluated. The sensitivity of Amor criteria was 88,5%, specificity was 91,9%; the positive predictive value was 93,3% and the negativepre-dictive value was 86,4%. For ESSG criteria, the sensi¬tivity was 86,6%, the specificity was 91,1% the positive predictive value was 92,5% and the negative predictive value was 84,3%. There was no significant difference between the sensi¬tivity, specificity and the predictive values between these two sets of diagnostic criteria. In conclusion, both sets of criteria have a great impor¬tance in the diagnosis of spondylarthropathies, and their performances are similar. Furthermore, because these criteria sets are complementary to each other, spondylarthropathies of unconfirmed diagnosis have to be evaluated and followed up with both sets of criteria.

Bu çalışmanın amacı son yıllarda ileri sürülen ve birçok araştırmaya konu olan Amor ve Avrupa Spondilartropati Çalışma Grubu (ESSG) Spondilartropati Sınıflandırma Kriterleri'nin değerini ortaya koymaktır. Araştırma GATA Haydarpaşa Eğitim Hastanesi Fiziksel Tıp ve Rehabilitasyon Kliniğinde gerçekleştirildi. Hastalar iki gruba ayrıldı. Birinci grupta spondilartropatili 157 hasta yer almaktadır. Spondilartropati dışında değişik romatizmal hastalıklar içeren 124 hastadan oluşan ikinci grup kontrol grubu olarak alındı. Bu kriterlerin sensibilite ve spesifitesini değerlendirmek için 24 parametre kullanıldı. Öte yandan iki tip kriterin negatif ve pozitif prediktif değerleri ve performansları saptandı. Amor Kriterleri'nin sensibilite ve spesifitesi sırasıyla %88.5 ve %91.9 olurken, pozitif ve negatif prediktif değerleri de %93.3 ve %86.4 olarak bulundu. ESSG Kriterleri'ne gelince sonuçlar şöyledir: sensibilite: %86.6; spesifite: %91.1; pozitif prediktif değer: %92.5 ve negatif prediktif değer: %84.3'tür. Bu iki tanı kriter seti arasında sensibilite, spesifite ve prediktif değerler açısından anlamlı farklılık bulunamadı. Sonuç olarak, her iki kriter setinin de spondilartropati tanısında önemleri büyüktür ve performansları birbirine eşittir. Ayrıca bu iki kriter seti birbirinin tamamlayıcısı olduğundan tanısı kesinleşmeyen spondilartropatilerin her iki kriter seti ile izlenmesi ve değerlendirilmesi gerekmektedir.
