The relationship between circulating immune complex and clinical and laboratory findings in patients with rheumatoid arthritis

Abstract
In this study, serum circulating immune complex (CIC) of 69 patients with rheumatoid arthritis (RA) and 28 healthy control group were investigated. Swollen joints, tender joints, morning stiffness, erythrocyte sedimenta¬tion rate (ESR), CRP, RF, Ig G, Ig A, Ig M, C3c, C4 val¬ues were determined in RA patients. Hand X-rays were staged according to Larsen scores. In result, we have found serum immune complex values were statistically higher than controls (p<0.001). Statistical regression analysis revealed that circulating immune complex val¬ues are related with the morning stiffness. There was a reverse relationship between circulating immune com¬plex andlgA levels. We also found that there was a posi¬tive relationship between circulating immune complex and CRP, systemic manifestations and functional stages. We did not find any correlation between immune com¬plex values and the radiological staging and also RF positivity although some authors had found out.

Bu çalışmada 69 romatoid artritli (RA) hasta ile 28 kişilik sağlık kontrol grubunun serumlarındaki immün kompleks (İK) değerlerine bakıldı. Romatoid artritli hastaların şiş eklem sayısı, hassas eklem sayısı, sabah tutukluğu, eritrosit sedimentasyon hızı, CRP, RF, Ig G, Ig A, Ig M, C3c, C4 değerleri belirlendi. Romatoid artritli hastaların el grafileri Larsen skorlamasına göre evrelendirildi. Sonuç olarak RA'lı hastaların immün kompleks değerleri ile en anlamlı ilişkinin sabah tutukluğu ile olduğu, ayrıca Ig ile ters yönde, CRP, sistemik tutulum varlığı ve fonksiyonel evre ile ise doğru orantılı bir ilişki olduğu görüldü. Literatürlerde gözlenenin aksine radyolojik evrelendirme ile immün kompleks değerleri ve romatoid faktör pozitifliği arasında bir ilişki gözlenmedi.
