Massive pleural effusion due to rheumatoid arthritis: A case report

Abstract

54-year-old male patient presented with right pleuretic chest pain and dyspnea. He was diagnosed of rheu¬matoid arthritis two years ago and was receiving indomethazine and salazopyrine since. Tap of the right massive pleural effusion revealed an exudate with pH 7.20, LDH 2646 IU/1, glucose 1 mg/1, negative rheu¬matoid factor, and mononuclear and mesothelial cells. On thoracoscopy, the pleura had a granulomatous appearance and pathological examination confirmed rheumatoid pleuritis. The effusion resolved rapidly with oral corticosteroid therapy.

Özet

54 yaşında erkek hasta üç haftadır ortaya çıkan sağ plöretik göğüs 'ağrısı ve nefes darlığı nedeniyle başvurdu. İki yıl önce romatoid artritis tanısı aldığı, indometazin ve salazopirin kullandığı öğrenildi. Sağ masif plevral effüzyon saptanan hastaya yapılan torasentezde glukoz 1 mg/1, LDH 2646 IU/1, pH 7.20 ve RF negatif, mononükleerve mesotelial hücreler içeren eksüdatifsıvı alındı. Torakoskopideplevrada granülomatöz görünüm izlendi, patolojik olarak romatoid plörezi tanısı doğrulandı. Oral steroid tedavisi ile plevral effüzyon kısa sürede kayboldu.
