Analysis of prenatal invasive procedures performed at Süleymaniye maternity hospital in terms of complication rates, diagnostic efficiency and cost

Abstract
Objective: The aim of our study was the determination of the most appropriate prenatal invasive procedure for our population by comparing the distribution, complications, outcomes and the cost-efficiency of the prenatal invasive procedures performed at the Süleymaniye Maternity Hospital. Materials and Methods: All pregnant women screened between November 2001 thru May 2004 at the Prenatal diagnostic unit of the Süleymaniye Maternity Hospital and found to have an estimated risk of aneuploidy greater than 1/300 were advised to undergo invasive diagnostic testing. Chorionic villus sampling (CVS), Amniocentesis (AS) and cordocentesis (KS) were done in women with a pregnancy of up to 15 weeks, between 15 and 21 weeks and greater than 21 weeks respectively. The database of the prenatal diagnostic unit was evaluated retrospectively to reveal the distribution of procedures, indications, outcomes and complications. The cost efficiency of the procedures was compared by calculating the cost of finding a positive case within each procedure. Results: A total of 6212 cases were scanned and a prenatal invasive procedure applied in 419 of these cases (6.7%). AS was done in 327, CVS in 33 and KS in 59 cases. The most frequent indications for AS and CVS were the high risk detected in the triple and first trimester screen tests respectively. KS was done predominantly for abnormalities detected by detailled ultrasonography. AS, CVS and KS revealed an abnormal karyotype in 19(19/327; 5.8%), 7(7/33; 21%) and 9(9/59; 15.25%) cases respectively. One patient aborted within 48 hours after AS (1/327; 0.03%) and another one after KS (1/59; 1.7 %) procedures. No fetal miscarriage was encountered after CVS. The total costs per application of AS, CVS and KS in our hospital are 230 000 000, 405 000 000 and 230 000 000 TL, respectively. Regarding the type of applied invasive procedure, costs per diagnosed karyotype anomaly are 10.744.000.000, 2.227.000.000 and 2.714.000.000 TL in AS, CVS and KS respectively. Conclusion: Despite the relatively high laboratory costs, applicability at the first trimester, lower complication rates, and high diagnostic efficiency renders CVS to be the most appropriate prenatal invasive procedure for our population.

Amaç: Kliniğimizde uygulanan tüm prenatal invasif girişimlerin dağılım, komplikasyon, sonuç, verim ve maliyetlerini karşılaştırarak kendi populasyonumuz için en uygun girişimi belirlemek. Yöntem: Süleymaniye Doğum ve Kadın Hastalıkları Eğitim ve Araştırma Hastanesi Prenatal Tanı Ünitesinde Kasım 2001-Mayıs 2004 tarihleri arasında taranan ve aneuploidi riski 1/300’ün üzerinde bulunan tüm gebelere prenatal invasif girişim önerildi. 12- 14 haftaya kadar Koryon villus biopsisi (CVS), 15-21 haftalar arasında amniosentez (AS) ve 21. haftadan sonra kordosentez (KS) uygulandı. Prenatal tanı ünitesi veritabanı incelenerek invasif girişimlerin sayısı, sıklığı, endikasyonları, sonuçları, komplikasyonları irdelendi. Maliyet açısından prosedürleri karşılaştırabilmek için her bir prosedürde pozitif bir olgu bulabilmenin maliyeti hesaplandı. Bulgular: Toplam tarama yapılan olgu sayısı 6212, prenatal invasif girişim yapılan olgu sayısı 419 (%6.7) dur. 327 olguya amniosentez, 33 olguya CVS, 59 olguya da KS uygulandı. Amniosentez en sık üçlü tarama testinde yüksek risk nedeni ile, kordosentez ultrasonografide belirteç görülmesi nedeni ile, CVS ise ikili taramada yüksek risk nedeni ile uygulandı. AS, CVS ve KS olgularında sırasıyla 19(19/327;%5.8), 7(7/33; %21) ve 9(9/59;%15.25) karyotip anomalisi saptandı. Eski intraamnial kanama saptanan (kahverengi amnios mayii) bir olguda AS’yi (1/327; %0.3) 1 olguda da KS’yi (1/59; %1.7) takiben 48. saatte amnial rüptür ve düşük gerçekleşti. CVS olgularında fetal kayıp olmadı. AS, CVS ve KS işlemlerinin hastanemizdeki toplam maliyetleri sırasıyla 230, 405 ve 230 milyon TL’ dir. İnvasif girişimin tipine göre tanı konulan her karyotip anomalisi başına maliyet AS, CVS ve KS için sırasıyla 10.744, 2.227, 2.714 milyar TL’dir. Sonuç: Göreceli olarak yüksek laboratuar maliyetine rağmen düşük tanı maliyeti, ilk trimesterde uygulanabilir oluşu düşük komplikasyon hızı ve yüksek verimi ile CVS bizim populasyonumuz için en tercih edilir prenatal invasif tanı girişimidir.
