Autopsy findings in fetal anomalies

Abstract
Objective: To evaluate the autopsy findings on fetal malformation cases. Methods: Forty-four cases of fetal malformations detected during autopsy of 147 fetal or neonatal death cases were evaluated retrospectively between 1998-2002 years. All cases are grouped according to maternal age, gestational week, fetal weight, gender, and clinical and pathologic diagnosis. Final diagnosis on autopsy is obtained after macroscopic and microscopic evaluation. Fetal malformation cases are grouped and discussed according to the distribution of maternal age, week of delivery, birth weight, and to the frequently seen malformation. Results: Fetal malformation was diagnosed in 44 out of 147 autopsy cases (30%). Nineteen of the cases were female, 25 of them were male. Most frequent malformations resulted from genitourinary system (12.2%), muscleskeletal system (10.2%), and central nervous system (8.1%). The mean maternal age of the index cases was 25.7±4.8 (19-40 years), the mean week of delivery was 28.1±6.34 (15-39 weeks). While genitourinary and muscleskeletal system malformations were common at younger maternal ages, genitourinary and central nervous system malformations were common in advanced maternal ages. Conclusion: Congenital malformations consist of a remarkable cause of the perinatal deaths. Perinatal autopsy is a leading method to clarify the cause of perinatal mortality, the recurrence risk of the disease and facilitate the management of the following pregnancy. A multidisciplinary approach concerning perinatology, genetic, neonatology, neonatal surgery and pathology teams may allow better diagnosis of the fetal malformations and their management.

Amaç: Fetal malformasyon olgularının otopsi sonuçlarını değerlendirmek. Yöntem: 1998-2002 yılları arasında otopsi yapılan 147 fetal ve neonatal ölüm olgusundan fetal anomali saptanan 44 olgu retrospektif olarak değerlendirildi. Olgular, anne yaşı, gebelik haftası, fetus ağırlığı, cinsiyet, klinik tanı ve patolojik tanılarına göre gruplandırıldı. Makroskopik ve mikroskopik değerlendirme sonucunda otopsi tanısı belirlendi. Otopsi bulgularına göre fetal anomalili olgular anne yaş aralığı, doğum haftası, doğum ağırlıkları ve sık görülen patolojilere göre gruplandırılıp irdelendi. Bulgular: Toplam 147 otopsi olgusunun 44’ünde (%30) fetal anomali saptandı. Olguların 19’u kız, 25’i erkek idi. En sık tespit edilen anomaliler, genitoüriner sistem (%12.2), iskelet sistemi (%10.2) ve santral sinir sistemi (%8.1) ile ilgili idi. Fetal anomalili olguların, anne yaş ortalaması 25.7±4.8 yıl (19-40 yıl), ortalama gebelik haftası 28.1±6.34 hafta (15-39 hafta) idi. Çalışmamızda, genç annelerde genitoüriner sistem ve iskelet sistemi patolojileri daha sık olarak saptanırken, ileri yaş grubundaki annelerde daha çok genitoüriner sistem ve santral sinir sistemi patolojileri tespit edildi. Sonuç: Doğumsal anomaliler perinatal dönemdeki ölümlerin önemli bir nedenidir. Perinatal otopsi ölü doğum, neonatal ölüm veya doğum komplikasyonu sonucu görülen ölümlerin sebebini ve hastalığın tekrarlama riskini belirlemede ve sonraki gebeliklerin takibinde önemli bir tanı yöntemidir. Bu konuda, perinatoloji, genetik, neonatoloji, çocuk cerrahisi ve patoloji klinikleri arasında multidisipliner bir yaklaşım ile fetal anomalilerin doğru tanısı ve daha başarılı tedavisi mümkün olabilir.
