Abstract

Objective: The purpose of this study is to evaluate the incidence, underlying obstetric risk factors, and perinatal outcomes of placenta previa. Methods: Seventy-nine patients with placenta previa were evaluated from 3828 of pregnancies in our clinic between July 2001 and July 2004. The demographic characteristics, gestational weeks, delivery modes, neonatal weight, apgar scores of 1st and 5th minutes, fetal sex, smoking, and the numbers of previous cesarean sections in the patients with placenta previa were evaluated. In addition, prenatal and early neonatal mortality were evaluated. Results: The incidence of placenta previa was found as 2.06%. The mean age of patients with placenta previa was 31.15±5.94 (18-46), gravidity 5.25±2.74 (1-12), parity 3.65±2.65 (0-10) and gestational weeks were 34.20±3.42 (25-39). Sixteen of the patients (%20.25) had previously cesarean section. Only three of the patients had been smoker during existing pregnancy. 93.75% of the patients delivered with cesarean section, and cesarean hysterectomy was performed in two patients due to massive bleeding. Twin pregnancy was found in one patient. Mean neonatal weight was 2444±75 (600-4200 g), the first and fifth minutes apgar scores were found as 4.96±2.35 (0-9) and 7.05±2.48 (0-10) respectively. 58.75% of the fetuses were male, 11 (13.7%) of the fetuses died during prenatal and early neonatal period. Conclusion: In this study, the incidence of placenta previa was found higher than previously reported literature. Because of the referral position of our hospital, some of the outcome measures were not satisfactory. High gravidity, parity, and the previous cesarean section are the most important risk factors for placenta previa. Prematurity, insufficient neonatal intensive care unit, and the delayed arrival time of the patients to our center were considered to be responsible for the lower apgar scores and increased fetal mortality.

Amaç: Plasenta previa olgularında insidansı, obstetrik risk faktörlerini ve perinatal sonuçları değerlendirmektir. Yöntem: Kliniğimizde Haziran 2001-Haziran 2004 tarihleri arasında doğumu gerçekleşen 3828 gebelikten, 79 plasenta previa olgusu retrospektif olarak incelendi. Olguların demografik özellikleri, gebelik haftaları, doğum şekilleri, neonatal ağırlıkları, 1. ve 5. dakika Apgar skorları, fetus cinsiyeti, sigara kullanımı, daha önceki doğum şekilleri ve geçirdiği sezaryen sayıları belirlendi. Ayrıca, prepartum ve postpartum erken neonatal mortalite oranları hesaplandı. Bulgular: Çalışmamızda, plasenta previa insidansı %2.06 olarak bulundu. Plasenta previa olgularında ortalama yaş 31.15±5.94 (18-46), gravida 5.25±2.74 (1-12), parite 3.65±2.65 (0-10) ve gebelik haftası 34.20±3.42 (25-39) olarak bulundu. Olguların 16’sı (%20.25) eski sezaryenli idi. Üç olgunun gebeliği süresince sigara kullandığı tespit edildi. Plasenta previa olgularının %93.75’inde doğum sezaryen ile gerçekleşirken, iki olguya aşırı kanama nedeniyle sezaryen histerektomi uygulanmıştır. Ortalama yenidoğan ağırlığı 2444±75 (600-4200 g), ortalama 1. dakika apgar skoru 4.96±2.35 (0-9) ve 5.dakika 7.05±2.48 (0-10) olarak bulundu. Bir ikiz gebelik tespit edildi. Fetusların %58.75’i erkek idi ve fetuslardan 11’i (%13.7) prepartum veya erken neonatal dönemde kaybedilmişti. Sonuç: Çalışmamızda, plasenta previa insidansı literatürden yüksek bulundu. Hastanemizin referans merkezi olması, sonuçlarımızın olumsuz çıkmasına etki etmektedir. Yüksek gravida, parite ve geçirilmiş sezaryen plasenta previa için en önemli risk faktörleridir. Bulduğumuz yüksek neonatal mortalite oranı ve düşük apgar skorunun, prematüriteden, yetersiz neonatal yoğun bakım hizmetinden ve olguların hastanemize geç intikal etmelerinden kaynaklanmış olabileceği düşünülmektedir.
