Angiographic control studies of 241 patients who underwent coronary artery bypass surgery

Abstract

Aim: We aimed to demonstrate the condition of arterial and venous grafts in symptomatic cases that had coronary artery bypass grafting (CABG) operation and to show whether patency rates of arterial grafts are higher in these cases. Material and Methods: In our study, control coronary angiography and ventriculgraphy was applied to 241 patients operated on CABG in various centers between Jan-2000 to Jan-2002. 220 internal thoracic artery (ITA) grafts, 442 saphenous vein grafts (SVG) and 17 radial artery (RA) grafts were evaluated, respectively. Results: The mean time of appearing complaints again after operation was 44±9.2 months. Subtotal occlusions in the venous grafts were 8.0%, total occlusion were 4.8% in the patients for 1mo-1years, postoperatively; the case was 9% for subtotal occlusion, 4.5% for total occlusion in the arterial grafts for the same period. Subtotal occlusions in the venous grafts were 26.7%, total occlusion were 23.2% in the patients for 7-10 years, postoperatively; the case was 30.3% for subtotal occlusion, 18.1% for total occlusion in the arterial grafts for the same period. Conclusion: The ratio of symptom-free conditions of the patient with CABG operations is 95% for the first year, 85% for 5 years, 65% for ten years, and 40% for 15 years, respectively. The reasons for these complaints in the early and late postoperative period are; inadequate revascularization, graft occlusion and perioperative myocardial damage. The use of alternative arterial grafts instead of venous ones seems to provide with CABG patients a long time symptom-free life. RA that alternative graft can be used as most available SVG.

Amaç: Koroner arter bypass operasyonu (KABG) geçirmiş olan semptomatik hastalarda kullanılan arter ve ven greftlerinin durumunu ve arteryel greftlerdeki açıklık oranının daha iyi olduğunu göstermeyi amaçladık. Gereç ve Yöntem: Çalışmamızda Ocak-2000 ile Ocak-2002 arasında farklı merkezlerde KABG geçirmiş 241 hastaya koroner anjiografi ve ventrikülografi uygulandı. 220 adet internal torasik arter grefti (İTA), 442 adet safen ven grefti (SVG) ve 17 adet radial arter(RA) grefti incelendi. Bulgular: Ameliyattan sonra şikayetlerin başlamasına kadar geçen ortalama süre 44±9.2 ay idi.1ay-1yıl arasındaki hastalar için venöz greftlerde parsiyel tıkanma %8.1, total tıkanma oranı %4.9, aynı dönemde arteryel greftlerde parsiyel tıkanma %6.8, total tıkanma oranı %4.5 idi. 7-10 yıl arasındaki hastalar için venöz greftlerde parsiyel tıkanma %26.7, total tıkanma %23.2, aynı dönemde arteryel greftlerde parsiyel tıkanma %24.2, total tıkanma oranı %18.1 idi. Sonuç: KABG geçiren hastalarda semptomsuzluk ilk 1, 5, 10 ve 15 yıl için sırasıyla %95, %85, %65 ve %40 oranındadır. Postoperatif erken ve geç dönem bu şikayetlerin nedenleri; yetersiz revaskülarizasyon, greft tıkanmaları ve perioperatif miyokard hasarıdır. Alternatif arteryel greftlerin venöz greftlerin yerine kullanılması KABG'li hastalar için daha uzun semptomlardan yoksun bir yaşam sağlayacaktır. SVG yerine RA alternatif greft olarak kullanılabilir.
