Chlamydia trachomatis specific antigen and antibody positivities in clinically diagnosed trachoma patients

Abstract
Trachoma has still been endemic in the Southeastern Anatolia of Turkey. In the present study, Chlamydia trachomatis elementary body antigens and anti-chlamydial IgG antibodies were investigated by immunofluorescence methods in the upper eyelid swab and serum specimens respectively, of 33 trachoma patients who were clinically diagnosed and followed by Ophthalmology clinics of Harran University, Medical Faculty in Şanlıurfa. The age ranges of the 16 male and 17 female patients, were between 14-80 years (mean age: 38.2 $\pm$ 13.4 years old), and they had not received any topical or systemic antibiotics during the last one month, although they had been previously treated with anti-chlamydial antibiotics. As a result, 20 (60.6%) of 33 eyelid samples were found positive for chlamydial antigens, whereas 25 (75.7%) serum samples were C.trachomatis IgG positive in titers above 1/3.2. The positivity rate of both C.trachomatis antigens and antibodies were 54.5% (18/33), while the negativity rate was 18.2% (6/33). Seven of the patients showed only seropositivity and 2 showed only antigen positivity. In 15 of 25 seropositive patients the IgG antibodies were specific for BDE, in 9 for CHIJ and in 1 for FGK serotypes. In conclusion, the antigen and/or antibody presence against C.trachomatis, in patients who have been diagnosed as trachoma clinically and treated with systemic/topical antibiotics previously, has been thought to be due to ineffective treatment, or failure of therapy due to antimicrobial resistance. These results also indicate that re-infections and/or recurrent infections were common in this region.

Trahom, yurdumuzun Güneydoğu Anadolu bölgesinde halen endemik olarak görülmektedir. Bu çalışmada, Harran Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı'na başvuran ve klinik olarak trahom tanısıyla takip edilen 16 erkek ve 17 kadın (ortalama yaş: 38.2 ± 13.4 yıl, yaş aralığı: 14-80 yıl) olmak üzere toplam 33 hastanın üst göz kapağı sürüntü örneklerinde Chlamydia trachomatis elementer cisimcik antijenleri ile serumlarında özgül IgG antikor varlığı immün floresan yöntemleriyle araştırılmıştır. Bu olgular daha önce anti-klamidyal tedavi almış olmalarına rağmen, son bir ayda herhangi bir topikal veya sistemik antibiyotik kullanmamışlardır. Çalışmamızda, 20 (%60.6) hastanın göz kapağı sürüntü örneğinde antijen pozitifliği, 25 (%75.7) hastada ise 1/32 ve üzeri titrelerde C.trachomatis IgG pozitifliği belirlenmiştir. C.trachomatis antijen ve antikorlarının birlikte pozitiflik oranı %54.5 (18/33), birlikte negatiflik oranı %18.2 (6/33) olarak saptanırken, hastaların 7'sinde sadece seropozitivite, 2'sinde sadece antijen pozitifliği bulunmuştur. Seropozitif olguların 15'inde BDE, 9'unda CHIJ ve 1'inde FGK serotipine karşı IgG varlığı mevcuttur. Sonuç olarak, klinikte trahom tanısı konularak önceden sistemik/topikal antibiyotiklerle tedavi edilen hastalarda C.trachomatis antijen ve/veya antikor pozitifliği, yetersiz tedavi ya da direnç nedeniyle tedavi başarısızlığını ve bu bölgede re-enfeksiyon/reküren enfeksiyon sıklığının yüksek olduğunu düşündürmektedir.
