Evaluation of the methods used for the serological diagnosis of Brucellosis

Abstract

Serological tests are the most widely used methods for the diagnosis of Brucella infections, since bacterial isolation techniques are time consuming. The conventional serological methods are the tube and slide agglutination tests, however enzyme immunoassays (ELISA) have gained importance recently as rapid diagnostic techniques. The aim of this study was to evaluate the performances of standard tube agglutination (STA, Wright test), slideagglutination (Rose-Bengal, RB) and ELISA-IgG and -IgM methods, in the serological diagnosis of brucellosis. For this purpose, Brucella antibodies have been searched in the serum samples of 184 patients (92 acute and 92 chronic brucellosis cases, of these 21 had positive blood cultures) and 20 healthy blood donors, who inhabit in the same region, as the control group. The positivity rates of STA (titers >1/160), ELISA-IgG and -IgM in the patient group were found 83.7%, 61.9% and 49.5% respectively, while RB positivities for Brucella abortus and B.melitensis were 61.9% and 67.9%, respectively. In the control group STA and ELISA tests showed negative results, but RB for B.aboiius and B.melitensis were positive in 25% and 30% of controls, respectively. Since ELISA and RB were found less sensitive than STA, it was concluded that STA is still the most reliable method for the serological diagnosis of brucellosis.

Özet

Brusellozis olgularının tanısında, bakteri izolasyon yöntemi zaman alici olduğundan serolojik testler sıklıkla tercih edilmektedir. Bu amaçla, lam ve tüp aglütinasyon testleri kullanılan konvansiyonel yöntemler olup, son yıllarda hızlı testlerden olan enzim immün yöntemleri (ELISA) de uygulamaya girmiştir. Bu çalışmada, brusellozisli hasta ve sağlıklı kontrol gruplarında standart tüp aglütinaşyonu (STA, Wright testi), lam aglütinasyonu (Rose-Bengal, RB) ve ELISA-IgG ve -IgM yöntemlerinin serolojik tanıdaki değerinin araştırılması amaçlanmıştır. Çalışmaya 92'si akut, 92'si kronik brusellozisli toplam 184 hasta (21'inin kan kültürü pozitif) ile aynı bölgede yaşayan sağlıklı 20 kan donörü dahil edilmiş ve deneklerin serum örneklerinde üç farklı yöntemle Brucella antikor varlığı araştırılmıştır. Hasta grubunda; 1/160 ve üzeri titrede STA, ELISA-IgG ve -IgM pozitifliği sırasıyla %83.7, %61.9 ve %49.5 olarak bulunurken, RB ile Brucella abortus için %61.9, B.melitensis için %67.9 oranında pozitiflik saptanmıştır. Kontrol grubunda ise; STA ve ELISA testleri ile negatif sonuç alınırken, RB ile B.abortus için %25, B.melitensis için %30 oranında pozitiflik belirlenmiştir. Sonuç olarak, RB ve ELISA yöntemlerinin tanısal duyarlılığı STA yöntemine göre daha düşük bulunmuş ve brusellozisin serolojik tanısında STA yönteminin halen güvenli bir test olma özelliğini koruduğu kanısına varılmıştır.
