Effect of Cigarette Smoke Extract on Phagocytosis of Staphylococcus aureus by Macrophages

Abstract
Staphylococcus aureus is one of the most important pathogen that causes community acquired and nosocomial infections worldwide. Phagocytosis by macrophages plays an important role in the fi rst line defense against infections caused by S.aureus. On the other hand, the conducted studies have indicated that cigarette smoke has negative effects on both innate and acquired immune responses. The aim of this study was to investigate the effects of cigarette smoke on macrophage viability and their capacity of S.aureus phagocytosis. For this purpose THP-1 cell lines (human leukemic monocyte cell culture) were used and after the differentiation of the cells with PMA (phorbol myristate acetate) treatment, the macrophages were exposed to cigarette smoke extract (CSE) for 2- and 4-hours at concentrations of 1%, 5%, 10%, 25%, and 50%. Afterwards, the cells were stained with propidium iodide and the viability of the cells was analyzed by a fl ow cytometer. Two different methods were used to investigate the effect of CSE on the phagocytosis of S.aureus. The fi rst one was the classical bacteriological method, in which macrophages were exposed to CSE for 2 hours in fi ve different concentrations and were infected with 100 MOI (multiplicity of infection) S.aureus. After 1 hour of incubation, macrophages were lysed with PBS-0.1% Triton X-100 and plated on Luria-Bertani (LB) agar following serial dilutions. Newly formed colonies were counted and the number of bacteria phagocytosed were evaluated as colony forming units (CFU). The second method for the detection of phagocytosis was fl ow cytometric analysis in which SYBR® Green-labeled bacteria were used. To confi rm that the macrophages were infected, bacteria were stained with SYBR® Green and macrophages were analyzed following infection via fl ow cytometry. Macrophages were exposed to 10% and 50% CSE and infected with bacteria stained with SYBR® Green. The level of phagocytosis was analyzed by fl ow cytometry in terms of median fl uorescence intensity. Macrophage death rate was 24% and 30% following 2-hour exposure to 25% and 50% CSE, respectively, while 4-hour exposure increased death rate to 38% and 51%, respectively (p< 0.001). At 10% and higher concentrations of CSE, cell death increased with an average of 1.5-fold between 2- and 4-hour exposures (p< 0.05). Macrophages were successfully infected (99.8%) with SYBR® Green-stained S.aureus. Phagocytosis of S.aureus by macrophages decreased signifi cantly upon exposure to 10% or more CSE concentrations (p< 0.0001). Median fl uorescence intensity, which indicates phagocytosed bacterial cell number, decreased with no statistical signifi cance when macrophages exposed to 10% and 50% CSE were infected with SYBR® Green-stained S.aureus. The results of this study revealed that, macrophage viability and phagocytosis of S.aureus were reduced depending on CSE concentration and time of exposure. In addition, it was shown that, SYBR® Green dye is a proper stain for the fl ow cytometric analysis of bacteria that were phagocytosed by macrophages

Staphylococcus aureus, tüm dünyada toplum ve hastane kaynaklı enfeksiyonlara neden olan önemli patojenlerden biridir. S.aureus'a bağlı enfeksiyonlara karşı ilk basamak savunmada makrofajlar tarafından fagositozu önemli rol oynamaktadır. Bununla birlikte yapılan çalışmalarda, sigara dumanının hem doğal hem de kazanılmış immün yanıt üzerine olumsuz etkileri olduğu belirlenmiştir. Bu çalışmanın amacı, sigara dumanı ekstraktının, makrofajların canlılığı ve S.aureus'u fagosite etme yeteneği üzerindeki etkisinin incelenmesidir. Çalışmada, THP-1 hücre dizisi (insan lösemik monosit hücre kültürü) kullanılmış ve hücreler PMA (phorbol myristate acetate) ile makrofajlara dönüştürüldükten sonra, sigara dumanı ekstraktı (SDE)'nın %1, %5, %10, %25 ve %50 konsantrasyonlarına 2 ve 4 saat süreyle maruz bırakılmıştır. Daha sonra propidium iyodür ile boyanan hücrelerin canlılıkları akım sitometri cihazı ile incelenmiştir. SDE'nin S.aureus fagositozuna etkisini incelemek için ise iki farklı yöntem kullanılmıştır. Bunlardan birincisi olan klasik bakteriyolojik yöntemde; makrofajlar 2 saat süreyle beş farklı konsantrasyonda SDE'ye maruz bırakıldıktan sonra 100 MOI (multiplicity of infection) S.aureus ile enfekte edilmiştir. Bir saat inkübasyondan sonra makrofajlar PBS-%0.1 Triton X-100 ile patlatılıp seri dilüsyon yapılarak Luria-Bertani (LB) agara ekilmiş ve ertesi gün koloni oluşturan birimler (CFU) sayılarak fagosite edilen bakteri sayısı belirlenmiştir. Fagositozun tespitinde kullanılan ikinci yöntemde, SYBR® Green ile işaretli bakteriler kullanılarak akım sitometrisi ile analiz yapılmıştır. Bu yöntemde öncelikle, makrofajların enfekte olduklarını kontrol etmek için bakteriler SYBR® Green ile boyanmış ve enfeksiyon sonrası makrofajlar akım sitometri yöntemiyle incelenmiştir. SDE'nin fagositoza etkisini tespit etmek için, makrofajlar %10 ve %50 SDE'ye maruz bırakıldıktan sonra SYBR® Green ile işaretli bakterilerle enfekte edilip, hücre içine alınan bakteriler akım sitometri yöntemiyle medyan fl oresan yoğunluk ölçülerek analiz edilmiştir. Yüzde 25 ve %50 SDE konsantrasyonlarında 2 saatlik maruziyet sonrası makrofaj hücre ölümü oranı sırasıyla, %24 ve %30 iken, 4 saatlik maruziyet sonrası sırasıyla %38 ve %51 düzeyine çıktığı gözlenmiştir (p< 0.001). Yüzde 10 ve üzeri SDE konsantrasyonlarında 2 ve 4 saatlik maruziyetlerde hücre ölümünde ortalama 1.5 kat artış saptanmıştır (p< 0.05). Green ile işaretli S.aureus'un kullanıldığı enfeksiyonlarda hücrelerin %99.8'inin enfekte edildiği gösterilmiştir. S.aureus'un makrofajlar tarafından fagositozunun %10 ve üzeri SDE konsantrasyonlarında azaldığı tespit edilmiştir (p< 0.0001). SYBR® Green ile işaretli S.aureus ile enfekte makrofajlarda SDE'nin %10 ve %50 olduğu durumlarda, fagosite edilen bakteri sayısını ifade eden hücre içindeki medyan fl oresan sinyalin, istatistiksel olarak anlamlı olmamakla birlikte azaldığı izlenmiştir. Sonuç olarak bulgularımız, SDE'nin makrofajlarda, konsantrasyona ve süreye bağımlı olarak hücre canlılığını ve S.aureus fagositozunu azalttığını ortaya koymuştur. Ayrıca, fagosite edilen bakterilerin akım sitometri yöntemiyle analizinde, bakterilerin boyanması amacıyla SYBR® Green boyasının başarıyla kullanılabileceği kanısına varılmıştır
