Investigation of BK virus antibody levels in pregnant women

Abstract
BK virus (BKV) is one of the human polyomavirus, generally acquired at the early years of life after an asymptomatic primary infection. It persists in the kidney tissue latently and can be reactivated following immunosupression. In view of the high seropositivity rate '(about 80%) of BKV in our population, the oncogenic potency of BKV and the increased sensitivity of pregnant women to viral infections and/or reactivations, the relation of the virus with transplacental transmission and congenital infection, has gained value. The aim of this study was to investigate the BKV antibody levels in 204 pregnant and 110 non-pregnant women aged between 18-36 years old, by hemagglutination inhibition (HI) test and to evaluate the results in view of the infection stages. As a result, total BKV seropositivity rate was found to be 82.4% (168/204) in pregnant and 80.9% (89/110) in non-pregnant women, while the rates of sensitivity to infection were 17.6% and 19.1%, the rates of passed infection and acquired immunity were 61.8% and 69.1%, and the rates of recently passed/recent infection and/or reactivation ($\geq$1/640 titers) were 20.6% and 11.8%, respectively. In statistical analysis, when each HI antibody titer rates were evaluated separately, there was no significant difference between the study groups by chi-square test, but when HI titers were considered as low ($\leq$1/20 titers), intermediate (1/40-1/320 titers) and high levels ($\geq$1/640), the difference between the groups in high antibody titers was found to be significant by importance test. In conclusion, it would be helpful to follow up the primary infections and/ or reactivations due to BKV by screening the pregnant women, although the relationship between BKV and congenital infections was not clear.

BK virus (BKV), genellikle asemptomatik primer enfeksiyon sonucu erken yaşlarda kazanılan, böbrek dokusunda latent olarak kalan ve immün süpresyon durumunda reaktive olan bir insan polyoma virüsüdür. Toplumumuzda daha önceden belirlenen yüksek seropozitiflik oranı (yaklaşık %80), virüsün sahip olduğu onkojenik potansiyel ve gebe kadınların viral enfeksiyon ve/veya reaktivasyonlara karşı gösterdiği duyarlılık dikkate alındığında, BKV'un transplasental geçiş ve konjenital enfeksiyon ile ilişkisi önem kazanmaktadır. Bu çalışmada, 18-36 yaş arasındaki 204 gebe ve 110 gebe olmayan kadında BKV antikor düzeylerinin hemaglütinasyon önlenim (HÖ) testi ile araştırılması amaçlanmış ve sonuçlar enfeksiyon dönemlerine göre yorumlanmıştır. BKV seropozitifliği gebe kadınlarda %82.4 (168/204) ve gebe olmayanlarda %80.9 (89/110) olarak belirlenirken, enfeksiyona duyarlı ($\leq$ 1/20 titreler) bireylerin oranı sırasıyla %17.6 ve %19.1, önceden geçirilmiş enfeksiyon ve kazanılmış bağışıklık (1/40-1/320 titreler) oranı sırasıyla %61.8 ve %69.1 ve yeni geçirilmiş/geçirilmekte olan enfeksiyon veya reaktivasyonu ifade eden 1/640 ve üzeri titrelere sahip kadınların oranı sırasıyla %20.6 ve %11.8 olarak saptanmıştır. Gruplar arasında BKV-HÖ titreleri tek tek dikkate alındığında saptanan fark ki kare testi ile anlamlı bulunmazken, titreler gruplandırıldığında yüksek antikor titrelerinde ($\geq$ 1/640), gruplar arasındaki farkın önemlilik testi ile anlamlı olduğu belirlenmiştir. Sonuç olarak, BKV'un transplasental geçişi ve konjenital enfeksiyonla ilişkisinin tam olarak bilinmemesine rağmen, gebelik sırasında BKV serolojisinin çalışılması ve enfeksiyon durumunun takip edilmesi yararlı olabilir düşüncesindeyiz.
