Case report : Ineffective treatment of a bacterial meningitis patient

Abstract
When an acute bacterial meningitis case is misdiagnosed and thus treated inappropriately, subsequent treatment and diagnosis could be immensely difficult for another following physician. With ineffective or low dose antibiotic regimens, not only do the findings of routine and cerebrospinal fluid (CSF) tests are changed, but also the chance of obtaining a positive culture is greatly reduced. In such cases, Gram stains and direct antigen tests gain much more importance. We here present a case of bacterial meningitis who has previously been misdiagnosed and treated with wrong antimicrobial agents.

Akut bakteriyel menenjitli bir hastanın yanlış tanı konulması ve hatalı tedavi edilmesi, bunu takiben başka bir klinisyenin doğru tanı ve tedavisini oldukça güçleştirmektedir. Etkisiz veya düşük dozda antibiyotik tedavileri, sadece rutin testleri ve beyin omurilik sıvısı (BOS) bulgularını değiştirmekle kalmayıp, aynı zamanda etkenin kültürde üreme olasılığını da büyük ölçüde azaltmaktadır. Bu gibi durumlarda, Gram boyama ve direk antijen testleri daha fazla önem kazanmaktadır. Bu raporda bu tip bir olgu sunulmaktadır.
