Investigation of Chlamydia pneumoniae and cytomegalovirus antibodies in patients with coronary heart disease

Abstract

This study was planned in order to investigate the proposed association of coronary heart diseases (CHD) with Chlamydia pneumoniae and Cytomegalovirus (CMV) infections, serologically. The levels of C.pneumoniae IgG/ IgA and CMV-lgG antibodies in sera of 112 patients with CHD and 28 patients with experienced acute myocardial infarction (AMI), ages between 40-60 years old, were evaluated together with the presence of the other risk factors, by comparing with a matched control group consisting of 100 healthy subjects. Microimmunofluorescence (MIF) method was used for the detection of C.pneumoniae IgG and IgA, and enzyme immunoassay for CMV-lgG antibodies. The liters of $\geq$1/256 and $\geq$1/32 were accepted as high antibody levels for C.pneumoniae IgG and IgA, respectively. The seropositivity rates of C.pneumoniae IgG in high titers were found 31%, 32% and 14% in CHD, AMI and control groups, respectively, while these rates were 24%, 36% and 16% for high titered C.pneumoniae IgA antibodies. The differences between both of the patient groups and controls for C.pneumoniae IgG and AMI group and controls for C.pneumoniae IgA were found to be statistically significant (p<0.05). The seropositivity rates of CMV-lgG were detected as 84%, 82% and 80% for CHD, AMI and control groups, respectively, with no significant difference between the groups (p>0.05). The mostly seen risk factor in the CHD patients with high C.pneumoniae IgG and IgA levels, was found as smoking habits with the rates of 87% and 74%, respectively, while these rates were 100% and 60% for AMI cases, followed by hypercholesterolemia and hypertension. Bu çalışmada, son yıllarda koroner kalp hastalıkları ile Chlamydia pneumoniae ve Sitomegalovirus (CMV) enfeksiyonları arasında ileri sürülen ilişkinin serolojik olarak araştırılması planlanmıştır. Bu amaçla, yaşları 40-60 yıl arasında değşen ve koroner kalp hastalığı (KKH) olan 112 hasta ile akut miyokard infarktüsü (AMI) geçiren 28 hastanın Chlamydia pneumoniae IgG/lgA ve Sitomegalovirus (CMV) IgG antikor düzeyleri, diğer risk faktörleri ile birlikte 100 sağlıklı kişiden alınan sonuçlarla karşılaştırılarak değerlendirilmiştir. C.pneumoniae IgG ve IgA testleri mikroimmünofloresan (MIF) yöntemiyle, CMV-lgG testi ise enzim immünoassay yöntemiyle çalışılmıştır. Antikor düzeyinin C.pneumoniae IgG için$\geq$1/256 ve IgA için $\geq$1/32 olması yüksek titreler olarak kabul edilmiş ve bu litrelerde C.pneumoniae IgG ve IgA seropozitiflik oranı KKH, AMI ve sağlıklı kişilerde sırasıyla %31 ve %24, %32 ve %36 ve %14 ve %16 olarak belirlenmiştir. Yüksek C.pneumoniae IgG düzeyi her iki hasta grubunda da, yüksek IgA düzeyi ise AMI geçiren hasta grubunda kontrol grubundan anlamlı olarak yüksek bulunmuştur (p 0.05). C.pneumoniae IgG ve IgA düzeyi yüksek KKH olgularında sırasıyla %87 ve %74 ve AMI olgularında %100 ve %60 oranları ile en sık rastlanan risk faktörünün sigara kullanma alışkanlığı olduğu tespit edilmiş, bunu hiperkolesterolemi ve hipertansiyon izlemiştir. Sonuç olarak, ateroskleroz gelişiminde kronik C.pneumoniae enfeksiyonunun bir risk faktörü olabileceği ancak patogenezdeki rolünün tartışmalı olduğu kanısına varılmıştır.
