IgG and IgG2 antibody responses to a pneumococcal capsular polysaccharide vaccine in patients with asthma and chronic obstructive pulmonary disease

Abstract
Streptococcus pneumoniae is the most common cause of community acquired pneumonia. Pneumococcal infections are responsible for substantial morbidity and mortality world-wide due to the increasing rate of penicillin resistance. Therefore pneumococcal vaccine is highly recommended to the high risk groups. In this study, we aimed to investigate the antibody response to Pneumo-23 vaccine in high risk groups especially for the patients with asthma and chronic obstructive lung disease (COLD) and to detect-IgG and lgG2 isotype antibody levels against the pneumococcal capsular polysaccharide antigens by ELISA method, in 41 patients aged between 44-85 years . Prevaccination IgG and lgG2 levels above threshold were determined in 83% and 51% of the patients respectively, whereas these rates have been detected as 93% and 90% respectively, one month after the vaccination. Immunology response to the vaccine (seroconversion) was detected in 41% of patients for IgG and in 63% of patients for lgG2. Anti-pneumococcal antibody mean levels were found to be significantly higher after vaccination (p<0.05), and there were no correlation between age, sex, underlying diseases and hospitalization condition (p>0.05). In conclusion, pneumococcal vaccine might be beneficial to increase the level of pre-existing pneumococcal antibodies and to protect the risk groups especially elderly people and COLD patients.

Streptococcus pneumoniae toplum kökenli pnömonilerde en sık saptanan etken olup giderek artan penisilin direnci nedeni ile oluşturduğu enfeksiyonlar önemli morbidite ve mortaliteye sahiptir. Bu nedenle yüksek risk gruplarında pnömokok aşısının kullanımı sıklıkla önerilmektedir. Bu çalışmada pnömoni açısından yüksek risk gruplarından seçilen astım ve kronik obstrüktif akciğer hastalığı (KOAH) tanısı konulan 41 hastaya Pneumo-23 aşısı uygulanmış ve pnömokokal kapsül polisakkarit antijenlerine karşı ELISA ile IgG ve lgG2 izotip antikorları araştırılmıştır. Buna göre aşı öncesi hastaların %83'ünde IgG ve %51'inde lgG2 tipi antikor seviyesi eşik değerin üzerinde saptanırken, bu oranlar aşılama sonrasında sırasıyla %93 ve %90'a yükselmiştir. Aşılama sonrasında serokonversiyon oranları anti-pnömokokkal IgG antikoru için %41, lgG2 için %63 olarak saptanmıştır. Anti-pnömokokal antikor seviyelerinin aşı öncesine göre anlamlı bir şekilde arttığı (p 0.05). Kronik akciğer hastalarında ve yaşlı bireylerde pnömokokal aşı, mevcut antikorları artırmada ve korunmada yararlı olabilir.
