Investigation of the proteinase activity of Candida albicans strains isolated from healthy subjects and patients with oral candidiasis

Abstract
This study was undertaken to investigate the relationship between the production of proteinase, a virulence factor of Candida albicans strains and development of oral candidiasis. The reliability of egg yolk agar which is used to determine the proteolytic activity of anaerobic bacteria, in detection of proteinases of C.albicans strains was also evaluated in the study. Thirty one C.albicans strains isolated from patients with oral candidiasis and 30 strains from healthy individuals were included. The proteinase activity of the strains was searched by using both bovine serum albumin (BSAA) and egg yolk ağar (EYA). Three of the strains (9.7%) isolated from patients showed two positive and 16 of them (51.6%) showed one positive, while 10 of the strains (33.3%) isolated from control subjects showed one positive proteinase activity on BSAA. A statistically significant difference was observed between the production of proteinase by C.albicans strains isolated from two groups (x2=4.78; p=0.029). When EYA was used, an agreement of 52.3% and a significant difference (x2=12; p= 0.0005) were found between the results. The sensitivity, specificity, positive and negative predictive values of using EYA were found to be 86.2%, 28.1%, 52.1% and 69.2%, respectively. According to our results, it was concluded that proteinases secreted by C.albicans play an important role in the pathogenesis of oral candidiasis and using EYA for the detection of proteinase activity of C.albicans strains doesn't seem to be an appropriate method.

Çalışmamız Candida albicans'ın virülans faktörlerinden olan proteinaz aktivitesi ile oral kandidiazis oluşumu arasındaki ilişkinin araştırılması amacıyla gerçekleştirilmiştir. Ayrıca bu çalışmada, anaerob bakterilerin proteolitik aktivitesinin gözlendiği yumurta sarılı ağarın, C.albicans türlerinin proteinaz aktivitesinin saptanmasında kullanılabilirliği de araştırılmıştır. Çalışmamıza oral kandidiazisli olgulardan izole edilen 31 ve sağlıklı bireylerden izole edilen 30 C.albicans susu dahil edilmiştir. Suşların proteinaz aktivitesi, sığır serum albumin ağar (SSAA) ve yumurta sarılı ağar (YSA) kullanılarak incelenmiştir. SSAA kullanıldığında, hastaların 3'ünde (%9.7) iki pozitif, 16'smda (%51.6) bir pozitif, kontrollerin ise 10'unda (%33.3) bir pozitif değerinde proteinaz aktivitesine sahip suşlar saptanmış ve her iki gruptan izole edilen suşların proteinaz üretimi arasında istatistiksel olarak önemli bir fark gözlenmiştir (x2=4.78; p=0.029). YSA kullanıldığında ise, sonuçların %52.3 oranında uyumlu olduğu görülmüş ve her iki ağar kullanılarak elde edilen sonuçlar arasında anlamlı bir fark bulunmuştur (x2=12; p=0.0005). YSA'ın duyarlılığı, özgüllüğü, pozitif ve negatif prediktif değerleri sırasıyla %86.2, %28.1, %52.1 ve %69.2 olarak belirlenmiştir. Sonuçlarımız ışığında, C.albicans'm proteinaz aktivitesinin oral kandidiazis patogenezinde önemli rol oynadığını ve C.albicans suşlarının proteinaz aktivitesinin saptanmasında YSA kullanımının uygun bir yöntem olmadığını düşünmekteyiz.
