Evaluation of the impact of successive sample examination on the diagnosis of tuberculosis by microscopy and Löwenstein-Jensen culture

Abstract

In this study we investigated the effect of examining three successive samples taken from the same patient on the laboratory diagnosis of Mycobacterium tuberculosis, by microscopy and culture methods. We retrospectively evaluated the results of culture and microscopic examination (by Ehruch-Ziehl-Neelsen method) belonging to 751 patients (601 patients were pulmonary, 150 were urinary tuberculosis cases) from whom M.tuberculosis was isolated by culture using Lowenstein-Jensen medium. Five hundred and fifty-eight of the pulmonary cases were diagnosed by sputum samples, 43 of them by early morning gastric juice (MGJ) while all urinary cases were diagnosed by urine samples. Totally, results of microscopy and culture studies of 1667 sputum, 129 MGJ arid 450 urine samples belonging to the patients who had given three samples in successive 24-^hour periods were evaluated, and as a result, second and third samples were designated to create a considerable increase in the efficiency of both methods for sputum and urine samples, while successive samples increased the efficiency of culture method but did not have any effect on microscopic examination for MGJ samples.

Bu çalışmada, Mycobacterium tuberculosis'ln laboratuvar tanısı için, bir hastadan ardışık olarak alınan üç örneğin mikroskopi ve kültür yöntemleri ile incelenmesinin bu yöntemlerin etkinliğine etkisi araştırılmıştır. Refik Saydam Hıfzıssıhha Merkez Başkanlığı, Tüberküloz Referans Laboratuvarı'nda Lowenstein-Jensen besiyeri kullanılarak yapılan kültür çalışması ile M.tuberculosis izole edilmiş olan 601'i akciğer, 150'si üriner tüberkülozlu olmak üzere toplam 751 hastaya ait kültür ve mikroskopi (Ehrlich-Ziehl-Neelsen boyama yöntemi ile) sonuçları retrospektif olarak değerlendirilmiştir. Akciğer tüberkülozu olan olguların 558'ine balgam, 43'üne açlık mide suyu (AMS) örnekleriyle, üriner tüberkülozlu olguların tümüne ise idrar örnekleri ile tanı konulmuştur. Üç gün ardışık olarak örnek alınan hastalara ait toplam 1677 balgam, 129 AMS ve 450 idrar örneği kullanılarak yapılan mikroskopi ve kültür sonuçları değerlendirildiğinde; balgam ve idrar örneklerinde ikinci ve üçüncü kez örnek incelenmesinin her iki yöntemin etkinliğini anlamlı bir şekilde artırdığı belirlenmiştir. AMS için ise, ardışık örnek kullanılmasının kültür yönteminin etkinliğini artırdığı ancak mikroskopinin etkinliğine anlamlı bir etkisinin olmadığı saptanmıştır.
