Intravenous immunoglobulins and hepatitis C virus transmission: A 5-years follow-up

Abstract
Intravenous immunoglobulin (IVIG) products which are derived from plasma of various donors, have been widely used in patients with hematologic and autoimmune diseases. Despite the recently developed effective viral inactivation methods, IVIG preparations may still have a risk for hepatitis C virus (HCV) transmission. In this study, a total of 32 patients (23 female, 9 male) ages between 1-18 years old (mean age: 5.8±3.6 years) who had an indication for IVIG administration and never been transfused by blood or any-blood derivatives in Pediatrics Clinics Haematology Unit of our hospital, were investigated in terms of HCV transfection after IVIG use. Three IVIG products prepared by different viral inactivation techniques including solvent detergent (S/D), pasteurization (P) and pH4+pepsin (pH4-P) methods, were included to the study. The cases were followed up during five years (at the beginning and 3rd, 9th, 24th and 60th months after IVIG administration) for the presence of anti-HCV antibodies, HCV-RNA and alanine aminotransferase (ALT) levels. Additionally, anti-HCV was searched in an IVIG sample which was randomly selected from the products used for each one of the patients. Before the IVIG therapy, all the patients were found to be anti-HCV negative, 21.9% were positive for hepatitis A virus (HAV) IgG and 31.3% were positive for anti-HBs (antibodies to surface antigen of hepatitis B virus). In the follow-up period after the first IVIG administration, anti-HCV was found to be positive in only one sample of a patient collected at the third month, and 2 of 32 (6.2%) IVIG samples. The clinical, biochemical and serological markers were found to be negative in terms of HCV transmission in these two cases as well as all the other cases, in the 5 years follow-up period. So these findings supported that anti-HCV positivities detected by enzyme immunoassay method, were "false positivities". No HCV-RNA positivities were detected by reverse transcriptase-polymerase chain reaction in none of the patients' sera and IVIG products. In conclusion, the IVIG products prepared by three different inactivation methods were thought to be safe by means of HCV transmission, however, they should be used carefully and the patients should be followed-up periodically.

Hematolojik ve otoimmün hastalıkların tedavisinde kullanıma giren intravenöz immünoglobulinler (IVIG), çok sayıda vericiden elde edilen bir plazma ürünüdür. Son yıllarda geliştirilen etkin viral inaktivasyon yöntemlerine karşın, IVIG'in halen hepatit C virus (HCV) bulaş riski taşıdığı bilinmektedir. Bu çalışmada, hastanemiz Çocuk Sağlığı ve Hastalıkları Klinikleri Hematoloji Ünitesinde IVIG kullanma endikasyonu konulan, daha önce kan ve kan ürünü almamış, 1-18 yaş arasındaki (ortalama: 5.8±3.6 yıl) 23 kız, 9 erkek olmak üzere toplam 32 olgu IVIG kullanımı sonrası HCV bulaşı yönünden araştırılmıştır. IVIG preparatı olarak, viral inaktivasyonun çözücü deterjan (S/D), pastörizasyon (P) ve pH4+pepsin (pH4-P) yöntemleriyle yapıldığı 3 ayrı ürün kullanılmıştır. Olgular IVIG öncesinde (0. gün) ve sonrasında 5 yıl süre ile (3, 9, 24 ve 60. aylarda) anti-HCV, HCV-RNA (18-24. ayda) ve karaciğer fonksiyon testleri (KCFT) yönünden izlenmişlerdir. Ayrıca, her olgu için kullanılan IVIG preparatlarından rastgele yöntemle seçilen birer üründe de (toplam 32 ürün) anti-HCV varlığı araştırılmıştır. IVIG öncesi, olguların %21.9'unda anti-HAV (anti-hepatit A virüs) IgG ve %31.3'ünde anti-HBs (anti-hepatit B virüs yüzey antijen) pozitif bulunurken, anti-HCV tümünde negatif olarak belirlenmiştir. IVIG sonrası izlemde, sadece bir hastanın üçüncü ay kontrolünde anti-HCV pozitif bulunmuş, ancak daha sonraki aylarda bu hastanın tüm örnekleri negatif olarak tesbit edilmiştir. Test edilen IVIG ürünlerinin %6.2'sinde (2/32) de anti-HCV pozitifliği saptanmış, ancak bu ürünlerin verilmiş olduğu hastaların 5 yıllık izlemi boyunca klinik, biyokimyasal ve serolojik belirleyiciler negatif seyretmiştir. Bu bulgularla, enzim immünoassay yöntemiyle saptanan anti-HCV pozitiflikleri "yalancı pozitiflik" olarak değerlendirilmiştir. Hiçbir örnek ve IVIG ürününde HCV-RNA pozitifliği saptanmamıştır. Sonuç olarak, kullanılan üç ayrı ticari IVIG preparatının güvenilir olduğu, ancak yine de kullanım endikasyonları ve olguların izleminde HCV bulaşı yönünden duyarlı olunması gerektiği kanısına varılmıştır.
