Evaluation of throat screening cultures in the children with hearing defects

Abstract

It has been thought that, the normal throat flora might be changed and colonization rate of the pathogenic bacteria might be high in people with hearing defects, depending on the tongue and jaw dysmötility and decreased swallowing activity. In this study, it was aimed to evaluate the throat screening cultures obtained from children with hearing defects attending to official same ages and socioeconomical status. In a period of two months (January-February 2000), the throat cultures of 104 hear defected students, ages between 8-15 years old (mean: 12.3±2.3years) and age matched 80 normal children were screened, whereas there were no clinical complaints in the children of both groups. -It was found that normal throat flora of hear defected children consisted of 4.8% coagulase negative staphylococci, 2.9% Streptococcus pneumoniae and 1.9% Corynebacterium.species, in addition to alpha hemolytic streptococci. Group A beta hemolytic streptococci (GABHS) were isolated from 16 (15.4%) of hear defected and 4 (5.0%) of normal children. There was no difference in terms of the members of normal throat flora between the groups, but statistically significant difference was detected between the isolation rates of GABHS (p<0.05). The antibiotic susceptibility tests of GABHS strains were performed by disk diffusion method against penicillin, cefuroxime, azithromycin and erythromycin, and all of 4 strains isolated from normal subjects were found to be susceptible to all of the tested antibiotics, while the sensitivities of 16 strains isolated from hear defected children were found to be 100% to penicillin and cefuroxime, and 87.5% and 81.3% to azithromycin and erythromycin, respectively.

İşitme engelli bireylerde çene, dil ve yutkunma hareketlerinin az olmasına bağlı olarak normal boğaz florasının farklı ve boğazda kolonize olan patojen sayısının daha fazla olabileceği düşünülmektedir. Bu çalışmada, Afyon ili MEB Sağırlar Ortaöğrenim Okulu işitme engelli öğrencilerinde boğaz tarama kültürlerinin değerlendirilmesi ve yaş ve sosyoekonomik düzeyleri aynı olan normal çocuklardan alınan sonuçlarla karşılaştırılması amaçlanmıştır. İki aylık bir dönemde (Ocak-Şubat 2000), klinik şikayeti olmayan 8-15 yaş (ört: 12.3±2.3) arasındaki 104 işitme engelli çocuk ile işitme fonksiyonu normal 80 çocuktan alınan boğaz tarama kültürleri incelenmiş ve işitme engelli çocukların normal boğaz florasında alfa hemolitik streptokoklara ilaveten %4.8 oranında koagülaz negatif stafilokok, %2.9 oranında Streptococcus pneumoniae ve %1.9 oranında Corynebacterium türleri saptanmıştır. Ayrıca, bu çocukların 16'smdan (%15.4) ve kontrol grubunun 4'ünden (%5.0) grup A beta hemolitik streptokok (GABHS) izolasyonu yapılmıştır. Normal boğaz florası elemanları açısından iki grup arasında önemli bir fark bulunmazken, GABHS izolasyon oranları arasındaki fark istatistiksel olarak anlamlı bulunmuştur (p<0.05). Çalışma gruplarından izole edilen GABHS suşlarınm penisilin, sefuroksim, azitromisin ve eritromisine karşı duyarlılıkları disk difüzyon yöntemi ile araştırıldığında ise, normal çocuklara ait 4 suşun hepsinin bu antibiyotiklere duyarlı olduğu, işitme engelli çocuklara ait 16 suşun penisilin ve sefuroksim duyarlılığının %100, azitromisin ve eritromisin duyarlılığının ise sırasıyla %87.5 ve %81.3 olduğu saptanmıştır.
