Etiological agents in bacterial meningitis and cerebrospinal fluid shunt infections and their antibiotic susceptibilities

Abstract
Identification of etiologic agents and their antimicrobial susceptibility profiles are important for establishing the treatment protocols and prevention strategies in meningitis and cerebrospinal fluid shunt infections. A prospective study was undertaken to determine the bacterial strains isolated from patients with meningitis and shunt infections and to investigate their antibiotic susceptibilities. In the period of October - May 2003, 50 patients who were hospitalized in Infectious Diseases, Pediatrics and Neurosurgery Departments of Medical Faculty, Atatürk University Hospital, Erzurum, Turkey, were included in this study. The isolates were identified by conventional methods, and the antimicrobial susceptibility tests were performed by disk diffusion method according to NCCLS guidelines. Of the 16 patients (32%), cerebrospinal fluid cultures yielded bacterial growth, and the distribution of the isolates were as follows; Pseudomonas aeruginosa (n: 4), methicillin sensitive Staphylococcus aureus (MSSA, n: 4), methicillin sensitive coagulase negative staphylococci (MSCNS, n: 3), Escherichia coli (n: 2), Streptococcus pneumoniae (n: 1), Enterobacter aerogenes (n: 1) and methicillin resistant S. aureus (MRSA, n: 1). No anaerobic strain was isolated. Both of the E. coli isolates produced extended spectrum beta-lactamase (ESBL). There were 3 P. aeruginosa strains producing both inducible beta-lactamase (IBL) and ESBL. Although carbapenems were the most active agents tested against Gram negative,,strains, vancomycin, teicoplanin and tetracyclines were found to be the most effective antibiotics against Gram positive strains.

Bakteriyel menenjitler ve santral sinir sistemi şant enfeksiyonlarında etiyolojik etkenin ve antimikrobiyal direnç profilinin belirlenmesi, klinik tedavinin seçilmesi, yönlendirilmesi ve korunma stratejilerinin aydınlatılmasında oldukça önemlidir. Bu çalışma, Mayıs-Ekim 2003 tarihleri arasında Erzurum Atatürk Üniversitesi T^p Fakültesi Enfeksiyon Hastalıkları, Pediatri ve Nöroşirurji servislerinde izlenen ve klinik, biyokimyasal, hücresel incelemeler sonucu akut bakteriyel menenjit veya şant enfeksiyonu tanısı konan 50 hastada, enfeksiyon etkeni ve antibiyotik direnç profilinin saptanması amacıyla prospektif olarak gerçekleştirilmiştir. İzolatlar geleneksel yöntemlerle tanımlanarak, antibiyotik duyarlılık testleri disk difüzyon yöntemiyle NCCLS kriterlerine göre değerlendirilmiş ve 50 hastadan 16'sının (%32) BOS kültüründe üreme tespit edilmiştir. Örneklerden; 4 Pseudomonas aeruginosa, 4 metisiline duyarlı Staphylococcus aureus (MSSA), 3 metisiline duyarlı koagülaz negatif stafilokok (MSKNS), 2 Escherichia coli, 1 Streptococcus pneumoniae, 1 Enterobacter aerogenes ve 1 metisiline dirençli S. aureus (MRSA) izole edilmiştir. Anaerop bakteri üremesi saptanmamıştır. R aeruginosa suşlarının 3'ünde hem genişlemiş spektrumlu beta-laktamaz (GSBL) hem de indüklenebilir beta-laktamaz (IBL) pozitifliği bulunurken, E. coli suşlarının her 2'sinde de GSBL pozitifliği tespit edilmiştir. Gram negatif bakteriler için meropenem ve imipenemin, Gram pozitif bakteriler için ise vankomisin, teikoplanin ve tetrasiklinin en etkili antibiyotikler olduğu belirlenmiştir.
