Saptanmasında Modifi ye Hodge Testi ve Carba NP Test for Detection of Carbapenemases in Enterobacteriaceae Isolates

Abstract

A rapid, practical, and accurate identifi cation of carbapenemase-producing Enterobacteriaceae isolates is crucial for the implementation of appropriate infection control measures and proper treatment of the infections. For this purpose, a large number of phenotypic test methods have been developed, although none has 100% sensitivity and specifi city. Variations in sensitivity and specifi city of these tests based on the type of beta-lactamase enzymes carried by that isolates might result in differences between regions and countries. The aim of this study was to compare the performances of widely used modifi ed Hodge test (MHT) and Carbapenemase Nordmann-Poirel (Carba NP) test in the detection of carbapenemases in Enterobacteriaceae family members. A total of 65 Enterobacteriaceae isolates (43 bla10 blasensitivity to at least one carbapenem (ertapenem, imipenem or meropenem), and carriage of carbapenemase gene confi rmed by polymerase chain reaction (PCR), were included in the study. Seventy-eight isolates showing decreased susceptibility to carbapenems but lacking carbapenemase genes were used as controls. All isolates were identifi ed by using conventional methods as well as automated BD Phoenix System (Becton Dickinson, USA). The antimicrobial susceptibility testing was performed using the same automated system, and was confi rmed by disk diffusion method. Results were evaluated according to the CLSI criteria. MHT was performed in accordance with the CLSI guideline, and Carba NP test was carried out by a modifi ed protocol. Instead of imipenem monohydrate, which was used in the original protocol, 6 mg/ml imipenem/cilastatin was used in the modifi ed protocol. In the study, MHT identifi ed 90.8% (59/65) of carbapenemase-producing isolates, while 93.9% (61/65) of the isolates were identifi ed by Carba NP test. With MHT, four Klebsiella pneumoniae producing OXA-48, one Escherichia coli producing IMP, and one K.pneumoniae producing NDM-1, and with Carba NP test, one E.coli and one K.pneumoniae producing OXA-48, one E.coli producing IMP, and one Enterobacter cloacae producing VIM could not de detected. Three OXA-48-producing isolates (two K.pneumoniae and one E.coli) yielded late and weak positive results with Carba NP test. MHT had false positivity for 31 isolates, while Carba NP test showed no false positivity. In comparison of the sensitivity and specifi city of the two tests, sensitivities were found to be similar although the Carba NP has a slightly higher sensitivity than the MHT (93.9% versus 90.8%, respectively; p= 0.754), Carba NP was found more specifi c (100% versus 60.3%, respectively; p< 0.0001). With Carba NP test, 26% of the isolates (n= 16) were positive within 15 minutes, and 85% (n= 52) were positive within the fi rst hour. It was concluded that, Carba NP test showed high sensitivity and specifi city than the MHT and the results can be obtained more rapidly for the presence of carbapenemases in Enterobacteriaceae. The use of MHT alone is not recommended to confi rm the presence of carbapenemases produced by Enterobacteriaceae. On the other hand Carba NP test can be used for this purpose, however molecular analysis should be considered for suspicious negative results

Enfeksiyonların uygun olarak tedavi edilebilmesi ve enfeksiyon kontrol önlemlerinin uygulanabilmesi için karbapenemaz üreten Enterobacteriaceae izolatlarının hızlı, pratik ve doğru bir şekilde tanımlanması çok önemlidir. Bu amaçla çok sayıda fenotipik test yöntemi geliştirilmiş olmasına rağmen, hiçbirisi %100 duyarlılık ve özgüllüğe sahip değildir. Testlerin duyarlılık ve özgüllüğü, çalışılan izolatların taşıdığı betalaktamaz enzim tiplerine göre değiştiğinden, ülkeler ve bölgeler arasında farklılıklar görülebilmektedir. Bu çalışmada, Enterobacteriaceae ailesi üyelerinde karbapenemazların saptanmasında yaygın olarak kullanılan modifi ye Hodge testi (MHT) ve "Carbapenemase Nordmann Poirel (Carba NP)" testlerinin performanslarının karşılaştırılması amaçlanmıştır. Çalışmaya çeşitli klinik örneklerden izole edilen ve en az bir karbapeneme (ertapenem, imipenem veya meropenem) azalmış duyarlılık gösteren ve karbapenemaz geni taşıdığı önceden polimeraz zincir reaksiyonu (PCR) ile saptanan 65 Enterobacteriaceae izolatı (43 blatir. Karbapenemaz geni saptanmayan fakat karbapenemlere azalmış duyarlılık gösteren kontrol grubu olarak 78 izolat kullanılmıştır. Tüm izolatlar konvansiyonel yöntemlere ilaveten BD Phoenix otomatize sistemi (Becton Dickinson, ABD) ile tanımlanmıştır. İzolatların antimikrobiyal duyarlılıkları, BD Phoenix otomatize sistemi ile saptanmış ve disk difüzyon yöntemi ile doğrulanmıştır. Sonuçlar CLSI kriterlerine göre yorumlanmıştır. MHT, CLSI'ın önerileri doğrultusunda yapılmış; Carba NP testi, modifi ye protokole göre uygulanmıştır. Orijinal protokolde kullanılan imipenem monohidrat yerine, modifi ye protokolde bildirildiği gibi, 6 mg/ml imipenem/silastatin kullanılmıştır. Çalışmamızda, MHT ile karbapenemaz üreten izolatların %90.8'i (59/65), Carba NP testi ile %93.9'u (61/65) tanımlanmıştır. MHT ile OXA-48 üreten dört Klebsiella pneumoniae, IMP üreten bir Escherichia coli ve NDM-1 üreten bir K.pneumoniae; Carba NP testi ile OXA-48 üreten bir E.coli ve bir K.pneumoniae, IMP üreten bir E.coli ve VIM üreten bir Enterobacter cloacae saptanamamıştır. Carba NP testinde, OXA-48 üreten üç izolatta (iki K.pneumoniae ve bir E.coli) geç ve zayıf pozitifl ik görülmüştür. MHT ile 31 izolatta yalancı pozitifl ik saptanırken, Carba NP testi ile yalancı pozitifl ik saptanmamıştır. İki testin duyarlılık ve özgüllüğü karşılaştırıldığında; Carba NP'nin MHT'den biraz daha yüksek olmakla birlikte, duyarlılıkları benzer (%93.9'a karşı %90.8; p= 0.754) bulunmuş, ancak Carba NP testinin daha özgül (%100'e karşı %60.3; p< 0.0001) olduğu belirlenmiştir. Carba NP testi ile izolatların %26'sında (n= 16) 15 dakikada, %85'inde (n= 52) ise ilk bir saat içinde pozitif sonuç alınmıştır. Sonuç olarak, Enterobacteriaceae üyelerinde karbapenemaz varlığının saptanmasında, Carba NP testinin MHT'e göre duyarlılık ve özgüllüğünün yüksek olduğu ve kısa sürede sonuç verdiği saptanmıştır. Enterobacteriaceae üyelerinde karbapenemaz varlığının doğrulanmasında MHT'nin tek başına kullanılmaması önerilmektedir. Diğer taraftan Carba NP testi bu amaçla kullanılabilir, ancak şüpheli negatif sonuçların moleküler yöntemlerle araştırılması gereği göz önünde bulundurulmalıdır
