Enfective endocarditis of the mitral valve and pacemaker lead in a case with congenitally corrected transposition of the great arteries

Abstract
Congenitally corrected transposition of the great arteries (L-TGA) is a rare form of congenital heart diseases. Infective endocarditis (IE) has been presented only in a few cases with L-TGA so far. Since tricuspid valve are exposed to high pressure and turbulent flow, this valve is more frequently affected in IE associated with L-TGA. Vegetation on a ventricular pacemaker lead together with mitral valve has not been reported before in the literature. In this case we aimed to present echocardiographic imaging of congenital and acquired anomalies in a patient with L-TGA which included patent ductus arteriosus, bicuspid pulmonary valve and multiple vegetations.
Özet
Büyük arterlerin konjenital düzeltilmiş transpozisyonu (L-TGA) konjenital kalp hastalıklarının (KKH) nadir görülen bir formudur. Şimdiye kadar sadece birkaç L-TGA’lı vakada enfektif endokardit (EE) bildirilmiştir. Triküspit kapağın yüksek basınç ve türbülan akıma maruziyeti nedeniyle L-TGA ilişkili EE vakalarında daha sık olarak bu kapak tutulur. Mitral kapak ile birlikte ventriküler pacemaker lead’i vejetasyonu daha önce literatürde bildirilmemiştir. Bu vakada biz L-TGA’lı bir hastada duktus arteriyosus açıklığı, biküspit pulmoner kapak ve çoklu vejetasyonu içeren, doğumsal ve edinsel anormalliklerin ekokardiyografik görüntülerini sunmayı amaçladık.
