A case whom presents with acute inferior myocardial infarction ECG but a severe stenosis in left anterior descending coronary artery

Abstract
ST elevation myocardial infarction is a clinic situation usually presents with total oclussion in the coronary artery. The clinic situation and treatment strategy change according to the ocluded coronary artery. The ECG at the presentation helps us to diagnose, to guess the ocluded coronary artery and to asses the treatment. However, sometimes the ECG can not be enough to guess the ocluded coronary artery. Here we present a case who came to the emergency department with a typically chest pain and an acute inferior myocardial infarction ECG patern but a severe stenosis in left anterior descending artery (LAD) in the coronary angiography.
Özet
ST elevasyonlu miyokard infarktüsü (MI) genellikle koroner arterde tam oklüzyon sonucu oluşan klinik bir durumdur. Klinik tablo ve tedavi stratejisi tıkanan koroner artere bağlı olarak değişkenlik gösterir. Başvuru anında çekilen EKG bize tanı koydurmada, lokalizasyonu öngörmede ve tedaviyi yönlendirmede yardımcı olan en önemli araçtır. Fakat bazen elektrokardiyografi (EKG) bize lokalizasyonu öngörmede yeterli olmayabilir. Biz burada acil servise tipik göğüs ağrısı ile gelen ve EKG’sinde akut inferior MI paterni izlediğimiz fakat yapılan koroner anjiyografisinde sol anterior inen arter (LAD) lezyonu saptadığımız bir vakayı sunduk.
