The follow-up of serum aminotransferase levels and investigation of hepatitis B virüs load in inactive HBsAg carriers

Abstract
The aim of this study was to follow-up the serum alanine aminotransferase (ALT) levels in inactive HBsAg carriers during one year period and investigate the association between hepatitis B virus (HBV) DNA levels detected at the end of the year. At May 2005, 61 patients with HBeAg negative/anti-HBe positive chronic HBV infection, followed in our viral hepatitis clinic were included to the study.The patients' ultrasonographic examination of the liver were normal, they had no history of taking alcohol or routine medication, were anti-HCV seronegative and had normal ALT levels during the last 6 months and at the beginning of the study. Serum ALT levels of rd th th patients were followed in the 3 , 6 and 12 months of the study, and blood HBV-DNA th levels were analysed quantitatively in 12 month. During the one year period 89% (54/61) of the patients yielded continously normal ALT levels, while 11% (7/61) showed at least one ALT value above the normal levels (ALT>1.2x). Total HBV-DNA positivity rate was found as 59% (36/61). In inactive HBsAg carrier group, -namely HBeAg negative and serum ALT levels constantly normal- 57.4% (31/54) of patients were HBV-DNA positive and 23 (42.6%) were negative. Amongst the HBV-DNA positive patients the viral load 5 4 were detected as 10M0 copies/ml in six (19.4%), and <10 copies/ml in 25 (80.6%) patients. In patients who had at least one ALT value above normal limits, 71.4% (5/7) of 5 them were found HBV-DNA positive; two with HBV-DNA values of >10 copies/ml and 4 5 three with values between 10 -10 copies/ml. In conclusion, although ALT levels may be normal, it should always be taken into consideration that more than half of inactive HBsAg carriers exhibited low level viral replication, thus HBV-DNA and liver enzyme levels should be monitored routinely in order not to miss the acute manifestations.

Bu çalışmada inaktif HBsAg taşıyıcılarında bir yıl boyunca alanin aminotransferaz (ALT) düzeylerinin izlemi ve yıl sonunda bakılan hepatit B virus (HBV) DNA düzeyleri ile ilişkilendirilmesi amaçlanmıştır. Çalışmaya, Buca Seyfi Demirsoy Devlet Hastanesi kronik hepatit polikliniğinde Mayıs 2005 tarihi itibarıyla takip edilen kronik hepatit B hastalarından; HBeAg negatif/anti-HBe pozitif, son 6 ayda bakılan iki ALT düzeyi normal, ultrasonografik incelemede karaciğerde yağlanma veya fokal lezyon bulunmayan, alkol veya düzenli ilaç kullanım öyküsü olmayan, anti-HCV negatif ve çalışma başladığında ALT düzeyi normal olan 61 hasta dahil edilmiştir. Çalışmanın başlangıcından itibaren hastaların ALT düzeyleri 3., 6. ve 12. aylarda izlenmiş ve 12.ayın sonunda kantitatif HBV-DNA analizi yapılmıştır. Bir yıl boyunca hastaların %89'unda (54/61) ALT düzeyi sürekli normal olarak seyrederken, %11'inde (7/61) en az bir kez ALT yüksekliği (ALT>1.2 kat) saptanmış ve toplam HBV-DNA pozitiflik oranı %59 (36/61) olarak belirlenmiştir. İnaktif taşıyıcı olarak kabul edilen HBeAg negatif ve ALT düzeyleri sürekli normal seyreden 54 hastanın 31 'inde (%57.4) HBV-DNA pozitif, 23'ünde (%42.6) ise negatif olarak tespit edilmiştir. HBV DNA'sı pozitif olan hastaların 4 5 4 altısında (%19.4) virus yükü 10 -10 kopya/ml, 25'inde (%80.6) ise 10 kopya/ml, üçünde ise 10MO kopya/ml olarak saptanmıştır. Sonuç olarak, inaktif HBsAg taşıyıcılarının yarısından fazlasında ALT düzeylerinin normal sınırlarda seyretmesine rağmen düşük düzeyde de olsa viral replikasyonun devam ettiği göz önünde tutulmalı ve akut alevlenmelerin kaçırılmaması için HBV-DNA ve karaciğer enzim düzeylerinin düzenli kontrolü yapılmalıdır.
