Obturatory abscess and pelvic pain caused by Brucella melitensis

Abstract
Brucellosis which is a systemic infection, is seen endemically in Turkey. Although there a lot of complications related to brucellosis, soft tissue involvement is rare. In this case report a patient with mature cystic teratoma and a Brucella related obturatory abscess, was presented. A 21 years-old female patient with bilateral pelvic pain was admitted to the hospital with complaints of weakness, backache and abdominal swelling. Pelvic examination revealed bilateral adnexial sensitive masses. There were no history of systemic complaints or other organ involvement. The patient had no history of contact with infected animals, intramuscular injections or consumption of non-pasteurized foods. A mass of 56 x 63 mm with regular margins and hyperechogenic segments at the left side was detected at ultrasonographic examination. Left ovarian cystectomy and mass excision at the left side were performed during laparotomy. Histopathological examination of the mass revealed chronic granulomatous inflammation characterized by central necrosis, histiocyte and lymphocyte infiltration and formation of giant cells, compatible with brucellosis. Brucella melitensis was isolated from the capsule of pelvic mass on the fifth day of culture in BacT/ALERT Selectlink (Organon Teknika, USA) system. Brucella Wright agglutination titer which was 1:80 at the admittance, rose to 1:2560 at the sixth week of the follow-up. The patient was treated successfully with the excision of the abscess and 6 weeks course of oral doxycycline and rifampicin. Brucellosis should be considered in the differential diagnosis of pelvic pain and obturatory abscess, especially in endemic areas for brucellosis.

Sistemik bir hastalık olan bruselloz ülkemizde endemik olarak görülmektedir. Brucella enfeksiyonu sırasında çok sayıda komplikasyonun ortaya çıkabilmesine karşın, yumuşak doku tutulumu nadirdir. Bu raporda, matür kistik teratomla birlikte aynı anda görülen Brucella etkenli bir obturator apse olgusu sunulmaktadır. İki taraflı pelvik ağrısı olan 21 yaşında kadın hasta, halsizlik, sırt ağrısı ve karında şişkinlik nedeniyle hastaneye başvurmuştur. Pelvik muayenede her iki alt kadranda hassasiyet gösteren kitle palpe edilmiştir. Hastada sistemik yakınmalar veya diğer organ tutulumlarıyla ilgili semptomlar bulunmayıp, kas içi enjeksiyon, hayvan teması ya da taze süt/süt ürünleri tüketimi öyküsü mevcut değildir. Ultrasonografik incelemede, sol adneksiyal alanda düzenli sınırlı, hiperekojen alanlar içeren 56 x 63 mm boyutlarında kitle saptanmıştır. Laparatomik sol over kistektomi ve sağ kitle eksizyonu yapılan hastanın patolojik incelemesinde, sol over matür kistik teratom ve sağ pelvik kitlenin Brucella enfeksiyonu ile uyumlu, santralde nekroz, histiosit, lenfosit ve histiositik dev hücre formasyonlarından oluşan kronik granülomatöz inflamasyon belirlenmiştir. Kist duvarından alınan dokunun kültüründe (BacT/ALERT 3D Selectlink, Organon Teknika, ABD) 5. günde üreyen bakteri Brucella melitensis olarak tanımlanmıştır. Başvuru sırasında 1/80 titrede pozitif olarak saptanan Brucella Wright aglütinasyon testi, 6 hafta sonra alınan serum örneğinde 1/2560 olarak tespit edilmiştir. Hasta, apsenin çıkarılmasını takiben 6 haftalık oral doksisiklin ve rifampisin ile tedavi edilmiştir. Sonuç olarak, pelvik ağrı ve obturator apse olgularında özellikle endemik bölgelerde bruselloz olasılığının düşünülmesi gerektiği kanısına varılmıştır.
