Comparison of Tigecycline and Vancomycin Activities in an In Vitro Biofi lm Model Generated with Methicillin-Resistant Staphylococcusaureus

Abstract
Today, the most common cause of bloodstream infections, which led to high mortality, prolonged hospitalization and increased costs are the intravenous catheters. Among the microorganisms associated with catheter infections, staphylococci took the fi rst place and because of their biofi lm-forming properties they cause serious problems in treatment and management of the patients. Although the drug of choice in the treatment of methicillin-resistant Staphylococcus aureus (MRSA) infection is vancomycin, its effect on the bacterial biofi lm is known to be low. Tigecycline, newly used in our country is a well tolerated glycylcycline antibiotic. In this study, we aimed to compare the effi cacy of tigecycline and vancomycin in an in vitro MRSA biofi lm model. The study consisted of 10 MRSA strains, which were detected as causative agents of catheter-related infections in our hospital. The methicillin resistance of the strains were performed by disk diffusion test with oxacillin (1 ?g) disks and the biofi lm forming capacity of the strains was evaluated using the Congo red agar method. The silicone disks with created biofi lm layer were exposed to tigecycline (2 mg/ml) and vancomycin (2 mg/ml) for 24 hours and for 5 days 4-hours per day in a model of antibiotic lock therapy. The present study showed that, after incubating the silicon discs in antibiotic solution for 24 hours, colony forming unit counts of MRSA decreased from 105 cfu/ml to 510 cfu/ml in the tigecycline group and from 105 cfu/ml to 3.800 cfu/ml in the vancomycin group and remained the same in the control (105 cfu/ml) group (p< 0.001). In the antibiotic lock therapy model, incubation with antibiotics for 4 hours per day, yielded that the average growth was 1.800 cfu/ml in the tigecycline group and 8.700 cfu/ml in the vancomycin group, which was statistically signifi cant (p< 0.001). No growth was detected in the tigecycline group (0 cfu/ml) while in vancomycin group number of colonies in second, thirth and fourth days were 2.000, 260, 80 cfu/ml, respectively, no growth was seen in the fi fth day. From the fi rst day until the fourth day tigecycline was statistically more effective than vancomycin (p< 0.001, p< 0.001, p< 0.001, p= 0.013, according to days respectively). As a result, it was determined that tigecycline showed a higher effect on MRSA biofi lm layer created on silicon discs and the results suggested that tigecycline might be a good alternative in the treatment of catheter infections

Günümüzde yüksek mortalite, uzun süreli hastanede yatış ve artan maliyete yol açan kan dolaşımı enfeksiyonlarının en sık sebebi damar içi kateterlerdir. Kateter ile ilişkili enfeksiyon etkenleri arasında stafi lokoklar ilk sırayı almakta ve biyofi lm oluşturma özellikleri nedeniyle hastaların tedavisi ve yönetiminde ciddi sorunlara yol açmaktadırlar. Vankomisin, metisiline dirençli Staphylococcus aureus (MRSA) enfeksiyonlarının tedavisinde seçilecek ilaç olmasına karşın, bakteriyel biyofi lm tabakasına etkisinin az olduğu bilinmektedir. Ülkemizde yeni kullanıma giren tigesiklin ise, geniş spektrumlu, iyi tolere edilebilen bir glisilsiklin antibiyotiktir. Bu çalışmada, bir in vitro MRSA biyofi lm modelinde, tigesiklin ve vankomisinin etkinliklerinin karşılaştırılması amaçlanmıştır. Çalışmada, hastanemizde kateter enfeksiyonu etkeni olarak izole edilen 10 MRSA suşu kullanılmıştır. Suşların metisin direnci oksasilin diski (1 ?g) kullanılarak disk difüzyon yöntemi ile; biyofi lm yapma özellikleri ise Kongo kırmızılı agar yöntemiyle araştırılmıştır. Silikon diskler üzerinde oluşturulan biyofi lm tabakası 24 saat ve antibiyotik kilit tedavisi modelinde beş gün boyunca günlük dört saat, tigesiklin (2 mg/ml) ve vankomisine (2 mg/ml) maruz bırakılmıştır. Çalışmamızda, silikon disklerin antibiyotik ile 24 saatlik inkübasyonu sonucu; koloni sayısı, tigesiklin grubunda 105 cfu/ml'den ortalama 510 cfu/ml'ye ve vankomisin grubunda ortalama 3.800 cfu/ml'ye düşmüş, kontrol grubunda ise aynı (105 cfu/ml) kalmıştır (p< 0.001). Antibiyotik kilit tedavi modelinde, günlük dört saat antibiyotikle inkübasyon sonrası; birinci günde tigesiklin grubunun ortalaması 1.800 cfu/ml, vankomisin grubunun ortalaması ise 8.700 cfu/ml olarak bulunmuş; iki grup arasında anlamlı fark saptanmıştır (p< 0.001). Tigesiklin grubunda ikinci günden itibaren hiç üreme saptanmamış (0 cfu/ml); buna karşın vankomisin grubunda koloni sayısı ikinci, üçüncü ve dördüncü günler sırasıyla 2000, 260, 80 cfu/ml olarak izlenmiş, ancak beşinci günde üremenin olmadığı görülmüştür. Buna göre ilk günden itibaren dördüncü güne kadar tigesiklinin vankomisinden istatistiksel olarak daha etkili olduğu saptanmıştır (günlere göre sırasıyla; p< 0.001, p< 0.001, p< 0.001, p= 0.013). Sonuç olarak, silikon disklerde oluşturduğumuz biyofi lm tabakasına tigesiklinin daha yüksek etki gösterdiği bulunmuş ve bu bulgular, kateter enfeksiyonlarının tedavisinde tigesiklinin iyi bir alternatif olabileceğini düşündürmüştür
