Evaluation of Sentinel Infl uenza Surveillance of the Last Two Seasons: 2013-2014 and 2014-2015

Abstract
Flu caused by infl uenza viruses, is a serious public health problem all over the world with its high morbidity and mortality. Therefore World Health Organization (WHO) regularly collects the results of national infl uenza surveillance, evaluates the results and shares them on international portal. Thus, it provides the possibility of rapid prevention and preparation of countries that needs to be taken on the fi ght against the epidemic fl u. Starting from 2004-2005 season until today the current fl u activity in our country is also followed in accordance with sentinel surveillance. The aim of this study was to evaluate the results of sentinel surveillance data obtained by National Infl uenza Reference Laboratory in Istanbul University Faculty of Medicine, in 2013-2014 and 2014-2015 seasons. For this purpose, nasal/nasopharyngeal swab samples taken from the patients diagnosed as infl uenza-like illness by the volunteer family physicians in Izmir, Istanbul, Antalya, Edirne and Bursa were included in the study. A total of 1240 samples were delivered to our laboratory in three days, in Virocult® transport culture medium according to cold chain rules. All the samples were studied by real-time polymerase chain reaction (Rt-PCR) according to the protocols of Centers for Disease Control and Prevention (CDC). In our study, the positivity rates of infl uenza viruses in 2013-2014 and 2014-2015 seasons were detected as 31.4% (202/641) and 44.4% (289/650), respectively. In 2013-2014 season, infl uenza A(H3N2) virus was the predominant type with a rate of 93.1% (188/202), and the rest was infl uenza B virus (14/202; 6.9%). In 2014-2015 season, infl uenza B virus has been dominated with a rate of 60.2% (174/289), and the rates of infl uenza A(H1N1)pdm09 and infl uenza A(H3N2) were 30.4% (88/289) and 9.3% (27/289), respectively. The fl u season in 2013-2014 has started at 48th week and peaked at 52nd week, while it was started later in the second week in 2014-2015 season and peaked at 10th-13th week. The lineage of the infl uenza B viruses isolated in both seasons were identifi ed as B/Yamagata. Antigenic characterization of infl uenza A viruses isolated in our laboratory was found compatible with the vaccine strains. In conclusion, surveillance studies are highly important for the determination of the effects of fl u on public health and identifi cation of the approaches for fi ghting with fl u. In this sense, infl uenza surveillance of the countries are required to implement more effectively in an expanded fi eld of scale

İnfl uenza viruslarının neden olduğu grip, yüksek morbidite ve mortalite oranları ile tüm dünyada ciddi bir halk sağlığı sorunu oluşturmaktadır. Bu nedenle Dünya Sağlık Örgütü (DSÖ) ulusal infl uenza sürveyans çalışmalarının sonuçlarını düzenli olarak toplamakta, değerlendirmekte ve uluslararası paylaşıma sunmaktadır. Böylece, ülkelerin grip salgınları ile mücadelede alması gereken önlem ve hazırlıkların daha hızlı bir şekilde gerçekleştirilmesi için olanak sağlanmaktadır. Ülkemizde de 2004-2005 sezonundan başlayarak günümüze kadar mevcut grip aktivitesi sentinel sürveyans uyarınca izlenmektedir. Bu çalışmada, İstanbul Üniversitesi İstanbul Tıp Fakültesi, Ulusal İnfl uenza Referans Laboratuvarının 20132014 ve 2014-2015 sezonlarındaki sentinel sürveyans bulguları değerlendirilmiştir. Bu kapsamda, sorumlu olduğumuz İzmir, İstanbul, Antalya, Edirne ve Bursa illerinde gönüllü aile hekimleri tarafından infl uenza benzeri hastalık tanısı konulan hastalardan nazal/nazofarengeal sürüntü örnekleri alınmış ve en geç üç gün içerisinde laboratuvarımıza ulaştırılmıştır. Soğuk zincir kurallarına uygun olarak Virocult® transport besiyeri içerisinde gönderilen 1240 örnek, CDC (Centers for Disease Control and Prevention) protokollerine uygun olarak gerçek zamanlı polimeraz zincir reaksiyonu (Rt-PCR) ile incelenmiştir. Çalışmamızda, 20132014 sezonunda %31.4 (202/641); 2014-2015 sezonunda ise %44.4 (289/650) oranında infl uenza virus pozitifl iği saptanmıştır. 2013-2014 sezonunda %93.1 (188/202) oranı ile infl uenza A(H3N2) baskın tip olarak bulunmuş; infl uenza B pozitifl ik oranı %6.9 (14/202) olarak saptanmıştır. 2014-2015 sezonunda ise %60.2 (174/289) oranı ile infl uenza B virusu baskın olmuştur. Bu sezonda infl uenza A(H1N1)pdm09 ve infl uenza A(H3N2) pozitifl ik oranları sırasıyla %30.4 (88/289) ve %9.3 (27/289) olarak saptanmıştır. 2013-2014 grip sezonu 48. haftada başlayıp 52-2.haftada pik yaparken, 2014-2015 grip sezonu daha geç olarak 2. haftada başlamış ve 10-13. haftalarda pik yapmıştır. Her iki sezonda da izolen edilen infl uenza B suşlarının Yamagata soyundan olduğu belirlenmiştir. Laboratuvarımızda izole edilen infl uenza A viruslarının antijenik karakterizasyonu aşı suşları ile uyumlu bulunmuştur. Sonuç olarak sürveyans çalışmaları, gribin toplum sağlığı üzerindeki etkilerinin ortaya çıkarılması ve grip ile mücadele yollarının belirlenmesi açısından oldukça önem taşımaktadır. Bu anlamda infl uenza sürveyansının, ülkeler ölçeğinde alanı genişletilerek daha etkin bir şekilde uygulanmasına gerek vardır
