Abstract

Combined Valve and Coronary Artery Surgery In our department 34 patients have undergone into valve replacement with coronary bypass surgery between 8 May 1990 and 18 De¬cember 1995 and the results of these cases have been evaluated. 22 patients had been operated for MYR+CABG; 8 patients for AVR+CABG; 3 patients for MVR+AVR+CABG; 1 patients for MVR+AVR+CABG+Tricuspid De VegaAnnuloplasty. In the postoperative early period two patients ■ have died as a result of low cardiac output, one patient has died because of myocardial in¬farction, one patient has died because of cerebrovascular bleeding (postoperative early mortality is % 11.7). In our late period follow-up we lost one patient because of the endocarditis of the prosthetic valve, we lost another on because of the thrombosis of the prosthetic valve (postoperative late period mortality is %5.8). We didn't record mortality and morbidity in any other patients. In the result of the postoperative sixth month reveal, in the functional capacities of the patients significant elevationts have been found compared to the preoperative values. Coronary bypass surgery accompanying valve replacements are among the complex operations of the cardiac surgery and we are in the belief that by the increasing experience in the following years, postoperative mortality and morbidity wul decrease. Özet

Kliniğimizde 8 Mayıs 1990-18 Aralık 1995 tarihleri arasında 34 hastaya kapak replasmanlan ûe birlikte koroner bypass cerrahisi uygu¬lanmış olup bu olguların sonuçları değerlendirilmiştir. 22 hastamıza MVR+CABG, 8 hastaya AVR+CABG, 3 hastaya MVR+AVR+CABG, 1 hastamıza ise AVR+MVR+CABG+Triküspit De Vega Anuloplasti ameliyatı uygulanmıştır. Postoperatiferken dönemde 2 hastamız düşük kalp debisi, 1 hastamız miyokard infarktüsü, 1 hastamız ise serebrovasküler kanama ne¬deniyle exitus olmuştur (Postoperattf erken mortalite %11.7). Geç dönem takiplerimizde 1 hastamızı prostetik kapak endokarditi, diğer bir hastamızı da prostetik kapak trombozu nedeniyle kaybettik (Postoperatif geç dönem mortalite 965.8}. Diğer hastalarımızda mortalite ve morbiditeye rastlanılmamıştır. Postoperatif 6. ayda yapılan değerlendirme sonucunda hastaların preoperatif değerlerine göre fonksiyonel kapasitelerinde belirgin artış olduğu görülmüştür. Kapak replasmanı ûe birlikte koroner bypass ameliyatları, kalp cerrahisinin kompleks operasyonalrmdan olup, ûeriki yıRarda tecrübenin artması Ûe postoperatif mortalite ve morbiditenin daha da azalacağı inancındayız.
