The effects of the severity of autonomous dysfunction in diabetic autonomous neuropathy on late potential parameters and the relationship between late potentials and complex ventricular arrhythmias

Abstract

AIM: The aim of this study has been to investigate the presence of ventricular arrhythmia in Type 11 diabetic patients with autonomous dysfunction, (who has arrhythmia and high risk of sudden death), to examine the effects of autonomous dysfunction on late potential (LP) parameters detected with signal averaged ECG and to determine a possible relationship between late potential presence and complex ventricular arrhythmia. METHODS: A total of 40 cases, of which 27 were women and 13 were men (with a mean age of 59.7±7.8), who had Type II diabetes were included in the study. Six cases in whom ischaemia was detected with exercise testing were excluded from the study. Autonomous dysfunction degrees were determined by applying cardiovascular reflex tests to 34 patients. Signal averaged ECG records were taken with the time-surface analysis method using standard orthogonal X, Y, Z derivations and 24-hour Holter monitoring was carried out. RESULTS: The presence of LP was detected in 5 of 21 cases (14.7%) with severe autonomous dysfunction and in 1 of 13 patients (2.9%) with slight dysfunction. There was no significant difference between the two groups. However, the fQRS time was found to be significantly longer in the group with severe dysfunction when compared to the group with slight dysfunction. During Hotter monitoring, complex ventricular arrhythmia was detected in 7 cases (20.5%) with severe dysfunction and in 3 cases (8.8%) with slight dysfunction. No significant difference was found between the two groups. CONCLUSIONS: As a result, although LP frequency in diabetic cases was higher than that in healthy individuals, no relationship was determined between the severity of diabetic autonomous neuropathy and LP. Prospective studies are needed to assess whether not complex ventricular arrhythmia, detected with Holier monitoring, has any prognostic value in patients with diabetic autonon neuropathy.

Özet

AMAÇ: Bu çalışmanın amacı aritmi ve ani ölüm riski yüksek, otonom disfonksiyonlu Tip II diyabetli hastalarda ventriküler aritmi varlığını araştırmak, otonom disfonksiyonunun sinyal ortalamak EKG ile saptanabilen geç potansiyel (GP) parametreleri üzerine etkilerim incelemek ve eğer varsa geç potansiyel varlığı ile kompleks ventriküler aritmi arasındaki ilişkiyi belirlemekti. YÖNTEM: Çalışmaya Tip II diyabet olan, 27'si kadın, 13'ü erkek, toplam 40 olgu (yaş ortalaması 59.7±7.8) alındı. Egzersiz testi ile iskemi saptanan 6 olgu çalışma dışı bırakıldı. 34 hastaya kardiyovasküler refleks testleri uygulanarak otonom disfonksiyon derecelen belirlendi. Standart ortogonal X, Y, Z derivasyonları kullanılarak zaman-alan analiz yöntemi ile sinyal ortalamak EKG kayıtları yapıldı ve 24 saatlik Holter monitörizasyonu uygulandı. BULGULAR: Ciddi otonom disfonksiyonu olan 21 olgunun 5' inde (%14.7), hafif disfonksiyonu olan 13 hastanın 1' inde (%2.9) GP varlığı saplandı. İki grup arasında anlamlı fark yoktu, sadece ciddi disfonksiyonu olan grupta GP parametrelerinden olan filtre edilmiş ORS (fQRS) süresi hafif disfonksiyonlu gruba göre anlamlı olarak daha uzun bulundu. Holter monitörizasyonda ciddi disfonksiyonlu 7 olguda (%20.5) ve hafif disfonksiyonlu 3 olguda (%8.8) kompleks ventriküler aritmi saptandı. İki grup arasında anlamlı fark bulunmadı. SONUÇ: Daha önce yapılan çalışmalarda, diyabetik olgularda GP sıklığı, sağlıklı bireylerden daha yüksek olarak bildirilmesine rağmen diyabetik otonom nöropatinin ciddiyeti ile GP arasında ilişki saptanmadı. Holler monitörizasyon ile saptadığımız kompleks ventriküler aritmilerin, diyabetik otonom nöropatili hastalarda prognostik öneminin olup olmadığını değerlendirmek için prospektif çalışmalara gerek vardır.
