Abstract
Plasma Lipoprotein (a) Levels in NIDDM Patients with and without Vascular Diabetic Complications Plasma concentrations of lipoprotein (a) [Lp(a)J, an independent risk factor for atherosclerosis, were measured in 59 non-insulin-dependent diabetes mellitus (NIDDM) patients with and without vascular complications, and 21 nondiabetic healthy subjects. The plasma Lp (a) levels were found to be significantly increased in the NIDDM patients (34.4 ± 28.7 mg/dl) compared with the healthy subjects (19.3 ±19.1 mg/dl; p<0.05).Plasma Lp (a) levels in NIDDM patients with diabetic vascular complications (40.1 ±24.3 mg/dl) were significantly higher than those of the NIDDM patients without diabetic vascular complications (24.8±20.7 mg/dl) and healthy subjects (p<0.05). There were significant correlations between plasma Lp (a) levels and apolipoprotein B (apo B) in all NIDDM patients (r:0.68). No correlation was observed between Lp (a) levels and age, sex, duration of diabetes, fasting blood glucose, he¬moglobin Aic, the mode of treatment, triglycerides, total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, and apolipoprotein AI levels in all patients. It was concluded that Lp (a) was an independent risk factor for angiopathy in NIDDM patients and the patients who have a high plasma Lp (a) concentration should be kept under strict glycemic control.
Özet
Ateroskleroz için bağımsız bir risk faktörü olan Lp (a) 'nın plazma konsantrasyonları tip II diabetes mellitus (DM) 'lu 59 hastada (25 erkek, 34 kadın; yaş ortalaması: 54.5±10.3 yıl) ve 21 sağlıklı bireyde ölçüldü. Plazma Lp (a) seviyelerinin diabetik hastalarda kontrol grubuna göre anlamlı olarak arttığı bulundu (Lp (a) seviyeleri sırasıyla 34.4±28.7 mg/dl ve 19.3±19.1 mg/dl; p<0.05). Vasküler komplikasyonlu diabetik hastalarda plazma Lp (a) seviyeleri komplikasyonsuz hastalara göre anlamlı olarak yüksekti (Lp (a) seviye¬leri sırasıyla 40.1 ±24.3 ve 24.8±20.7 mg/dl; p<0.05). Diabetik hastalarda plzma Lp (a) seviyesi ile apolipoprotein B arasında an¬lamlı pozitif ilişki saptandı (r: 0.68). Plazma Lp (a) seviyeleri ile yaş, cins, diabet süresi, açlık kan glukozu, hemoglobin Alc, ve insulin ve oral antidiyabetik kullanımı, diğer lipidparametreleri (trigliseridler, total kolesterol, LDL-kolesterol, HDL-kolesterol, apo Al) arasında bir ilişki tesbit edilmedi. Sonuç olarak tip II DM'lu hastalarda Lp (a) 'nın anjiopati için bağımsız bir risk faktörü olduğu ve yüksek plazma Lp (a) konsantrasyonu olan diabetik hastaların sıkı glisemik kontrol altında tutulması gerektiği kanaatine varıldı.
