A new closure device for femoral artery after coronary intervention

Abstract

After invasive cardiology, manual removal of the sheath and prolonged manual compression to seal the arterial puncture after sheath removal carries a risk of local complications and prolongs the patient's hospital stay. AIM: The aim of this study was to investigate the feasibility of a novel sheath-removal and closure device for invasive coronary procedures. The results from a new device, "Prostar" (percutaneous vascular surgical device), were compared against the results of manual compression in achieving hemostasis. METHODS: Prostar was used after invasive coronary procedures with 25 patients. The complication rate and post-procedure immobilization period were compared with a control group of 20 patients, for whom manual compression was used. FINDINGS: The success rate for the 25 Prostar procedures was 92% and the patients were mobilized without complication after a mean time of 139,68±139,61 seconds. This was significant compared to the control group immobilization lime of 477,00±125,03 seconds. CONCLUSIONS: The Prostar vascular closure device was a safe and feasible method of securing arterial hemostasis after sheath removal. The post-procedure immobilization time was significantly less than for the control group (p< 0.001). This could be a better alternative to manual compression after intervention.

Girişimsel kardiyolojide, işlemlerden sonra kılıfların femoral arterden çıkarılması sırasında el ile yapılan kompresyon lokal komplikasyon riski taşımakta ve daha sonra hastaların mobilizasyonları için uzun zaman gerekmekledir. AMAÇ: Bu çalışmada amaç, koroner girişimlerden sonra femoral artere yerleştirilen kılıfların çıkarılmasında yeni bir yöntemin klinik güvenirliğini araştırmaktır. Yeni bir alet "Prostar" (perkütan vasküler cerrahi aleti) ile sağlanan hemostaz, manuel kompresyon ile karşılaştırılmıştır. YÖNTEM: Koroner arter girişimini takiben 25 hastada Prostar ile kapatılma işlemi yapılmış ve 20 olgudan oluşan kontrol grubu ile yürüme zamanı ve komplikasyonlar açısından karşılaştırılmıştır. BULGULAR: Toplam 25 femoral arteri kapatma işlemi %92 başarı ile uygulandı. Bu grup hastalarda ortalama yürüme zamanı 139,68±139,61 dk idi. Belirgin bir komplikasyon izlenmedi. Manüel kompresyon yapılan kontrol grubunda ortalama yürüme zamanı 477,00±125,03 dk idi. SONUÇ: Arteriyel hemostaz sağlamak amacıyla kullanılan vasküler kapama aleti başarı ile uygulanmış ve yürüme zamanı, kontrol grubunun yürüme zamanı ile karşılaştırıldığında her iki grup arasında anlamlı farklılık izlenmiştir (p<0.001). Bu yöntem, girişimlerden sonra el ile kompresyona alternatif olabilir.
